0010049723 | 2014-08-25 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed that Whipstock Ohio Drilling Rig #1 had ran and cemented 13.625 inch diameter intermediate casing at 1822 feet and completed BOP testing. |
0265242253 | 2014-09-02 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed that Whipstock Ohio Drilling Rig #1 had set their 2nd intermediate casing string and was in the process of stacking the rig in preparation to drill 4 conductor holes on the pad. Whipstock will commence drilling on the 34059244020000 Rector East 14H once conductors are set. |
0377475195 | 2015-04-29 | Preliminary Restoration | No Violations | On the day of my inspection accompanied by Southeast Regional Supervisor Brent Bear, the site passed preliminary restoration according to ORC 1509.072A. |

0736289344 | 2018-11-09 | Completion Testing | No Violations | Status check inspection found the well drilling and completion process is complete and the completion plugs have been drilled out. A stand alone snubbing unit service rig owned by Deep Well Services recently completed the work on the well and is now rigged up on the # 14 well on the location. The well has 2.875 production tubing set in the well bore to a depth of 7,952. The well will not be put into production until the remainder of the 5 wells on location have been completed. The said well was stimulated in 73 stages with a bottom toe sleeve set in the 5.5 diameter production string at 19,176. Crews are on site installing the production equipment and (2) separate tank batteries have been set. |
0746832466 | 2019-02-04 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead and a plunger lift system. The plunger lift system was plumbed up, but the wells were flowing without the assistance of the plunger. Production tubing valves were in the open position. Gauges were present on all casing strings and they read as follows, 13.375-inch diameter surface casing had 285 pounds per square inch, 9.625-inch diameter intermediate had 583 pounds per square inch, 5.5-inch diameter production casing had 2900 pounds per square inch, and the production tubing had 2900 pounds per square inch. The production equipment consisted of ten 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Wise tanker truck, USA Compressions, and Ascent Company well tender were all on location. I called Jarod Cogar with Ascents production to make him aware of the pressures found on the 9.625-inch diameter by 5.5-inch diameter annulus and stated that our records indicate that 18.625-inch diameter surface casing was ran on these wells and there were not any gauges present at surface to monitor pressures. I asked that Ascent look into getting a gauge and pressure relief valve assembled on the 18.625-inch casing. |
1011747535 | 2018-10-03 | Completion Testing | No Violations | During my inspection I witnessed stage 33 being completed. Operations were witnessed within the data van along with Ascent Company Man Doug Wilson. The top perforation within the 5.5-inch diameter production casing was at 14,059 feet and the bottom perforation was at 14,180 feet. Frac Tech dropped the frac ball, pumped a spacer, pumped 3000 gallons of hydrochloric acid, 209,785 pounds of 40/70 mesh sand, and 40,220 pounds of 100 mesh sand. Annular pressure monitoring was present on the screen within the van and average pressure was 220 pounds per square inch. |
1264741582 | 2014-08-20 | Casing String Witnessed | No Violations | On the day of my inspection I witnessed that Whipstock Ohio Drilling Rig #1 drilled a 23 inch diameter hole to the depth of 505 feet and ran 481 feet of 18.625 inch 87.5#/ft. J-55 casing and stabbed the float shoe with 4.5 inch diameter drill pipe while trying to break circulation their pits ran dry. Once the pits were replenished Whipstock began pumping again and broke circulation with 89bbls of fresh water. Baker Hughes was on site and pumped 36bbls of gel, 10bbls of fresh water and cemented casing with 480 sacks of Class A lead cement with 0.05% Static Free, 1% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, 0.5% BA-10A. Lead cement was followed 123 sacks of Class A tail cement with 0.05% Static Free, 3% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, and 0.5% BA-10A. Displaced cement with 6.8bbls of fresh water. Had good cement returns of 30bbls to surface. |

1577294509 | 2019-04-30 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead. Production tubing valves were in the open position. Pressure monitoring was present the 13.375-inch diameter intermediate casing had 326 pounds per square inch, 9.625-inch diameter intermediate had 511 pounds per square inch, 5.5-inch diameter production casing had 2160 pounds per square inch, and the production tubing had 1600 pounds per square inch. |
1710390716 | 2016-09-07 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed no current activity and the drilling permit expires on 12/17/16. The last activity was on 8/31/14 with the 9.625 inch intermediate being set at 6771feet. The cellar has been filled with pea gravel to the surface with a 6 inch polyvinyl chloride snorkel for water extraction. |
1895897610 | 2017-08-07 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed there was no change in the status of the well since the last inspection on September 19, 2016 with the 9.625 inch diameter intermediate secured with a bolted night cap. There is no production equipment, flowline or salesline in place and the location is shared with the Mohorovich 1S, 2S, 3S, 4S and Rector 14H. |
