Tools were pumped down with 100 barrels of produced water at 14 barrels per minute with 5600 pounds per square inch. Nine Wireline successfully set the plug at 11202 feet and perforated the casing with 36 shots from 11175 feet to 11075 feet.

1131465152 2016-11-10 Completion Testing No Violations On the day of my inspection I witnessed the drill out operations were completed as Deep Well Services Rig #4 was rigging down preparing to move off location. I completed walk around with Consultant Cristo Giannareas revealing that secondary containment was being repaired and replaced by Grady Rentals along with cleaning of location. Halliburton Services are scheduled to move back in to complete the frac operations of the 8H and 10H next week.
1420568341 2014-12-30 Casing String Witnessed No Violations I was onsite to witness the cementing of the 13.375 surface casing. Halliburton has rigged up there equipment to cement the well. They used 1230sks of Halcem system cement on the well. The cement job was a success. We received 15bbls of cement back to surface.

Tetra Technologies was onsite setting flowback equipment with flowback to begin on Friday. Production equipment included a sand trap, a line heater and a GP Unit. Also present 8-400 barrel tanks within containment, a horizontal heated separator, 2- vapor recovery towers, 2-compressor units, a flare stack and a gas combustor all shared with the Abe Troyer 2H, 6H, 8H and 10H. I met with Consultant Cristo Giannareas and completed walk around revealing installation of production equipment was on going.
1914308213 2016-11-04 Completion Testing No Violations On the day of my inspection I witnessed that Deep Well Services Rig # 4 (DWS) was tripping in the wellbore preparing to drill out the kill plug set at 7865 feet. Upon completion DWS will drill out remaining frac plugs and run 2.375 inch production tubing. I completed a walk around with Consultant Will Ochampaugh revealing proper secondary containment was in place and maintained.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of eight 400-barrel tanks, two vapor recovery towers, five sand traps, five gas production units, one compressor, one vent scrubber, and one combustor. Identification was located on the well. Upon entering the access road, a slip was noticed developing on the left side of the access road at the point of entry and an additional slip had developed on the stack pile. I made ODNR Engineer Mark Atkinson aware of my findings.
1936952749 2019-05-20 Production Wells No Violations The purpose of my inspection was a status check. On 4/25/2019 inspection found two slips have developed along the access road and on the topsoil stockpile. I contacted ODNR horizontal well pad Engineer Mark Atkinson and made him aware of the problem areas. During my inspection today the slips remain but have not increased in severability. The current slips will not effect pad stability and performance.
