0237534521 2018-10-04 Work Over Reconditioning No Violations The purpose of my inspection was a status check. On 9/26/18 Chesapeake Production Engineer contacted me and stated that the McClain 1H was being serviced. The well took on a large column of fluid that the plunger lift could not lift from the production tubing. A wireline was utilized to attempt to remove the seating nipple within the tubing and part of the wireline was lost. An additional wireline was tripped back in hole and 700 feet of that wireline was lost. A coil tubing unit was utilized in attempt to fish out the lost line within the tubing and the coil unit became stuck and was cut off at surface. Stone Well Service Rig 15 was rigged up on location and removed the lost line and coil tubing along with the production tubing from the 5.5-inch diameter casing. During my inspection Stone Well Services was on location with rig number 15. I spoke with Wallace the Chesapeake Company Man on location and he stated that they are waiting for a new string of production tubing to install and the well will be put back into production. A small area of gravel was saturated with oil on the pad beside the well and Wallace stated that they are going to be removing the contaminated gravel and replacing it with new gravel.
0545906019 2015-01-11 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Express Energy Services had drilled a 26 inch diameter hole to the depth of 120 feet hitting water at 105 feet and ran 120 feet of 20 inch conductor pipe. Express gained circulation and cemented pipe with 230 sacks Class A cement with 2% CaCl and .5#/sk. of floseal. Displaced cement with 39bbls of fresh water and had good cement returns of 18bbls to surface.

The production equipment included 12-400bbl tanks within containment, 2-NGL tanks, a vapor recovery tower, 2 low pressure horizontal heated separators, a vertical volume pot, an oil stabilizer unit and a gas combustor all shared with the 2H, 3H, 5H, 7H, 8H, 9H and 10H. Also present for the well was a GP unit and sand traps planned for installation are currently not on site. The well was equipped with a frac valve in the closed position and drill out is scheduled to begin the week ending 3/26/16.

Wellheads are in place with frac stacks to be installed tomorrow and Performance Technologies rigging up with planned frac commencement on Friday.

The production equipment included 12-400bbl tanks within containment, 2-NGL tanks, a vapor recovery tower, 2 low pressure horizontal heated separators, a vertical volume pot, an oil stabilizer unit and a gas combustor all shared with the 2H, 3H, 5H, 7H, 8H, 9H and 10H. Also present for the well was a line heater, a GP unit and a sand trap. The well is now online and was equipped with a production tree and valves in the open position. Gauges present read 2765psi on the tubing and 2975psi on the casing with annular pressure monitoring in place.

During my inspection the well was equipped with a plunger lift system with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of twelve 400 barrel tanks with one vapor recovery tower, eight sand traps, eight gas production units, two heated horizontal separators, and one combustor. Identification was present and production records were current. A Chesapeake contractor was on location preforming maintenance on the gas production unit.

Also present for the well was a line heater, a GP unit and a sand trap. The well was equipped with a production tree with the valves in the open position. Gauges present read 940 pounds per square inch on the tubing and 1245 pounds per square inch on the casing with annular pressure monitoring in place. I met with lease operator Corey Shriner as annular pressure on the 9.625 inch casing was at 1480 pounds per square inch. Mr. Shriner relieved the pressure to 200 pounds per square inch and was instructed to keep pressure under 606 pounds per square inch to be compliant with OAC 1501:9-1-08 (N).

The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 531 pounds per square inch, the 5.5-inch diameter production casing had 320 pounds per square inch, and the 2.875-inch diameter production tubing had 180 pounds per square inch. The production equipment consisted of twelve 400-barrel tanks with one vapor recovery tower, eight sand traps, eight gas production units, two heated horizontal separators, and one combustor. Identification was present and production records were current.
2032354800 2015-01-30 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the 9.625 inch intermediate casing. I was informed by Chesapeake consultant, Matt Whitehead, that the Nomac 72 rig had drilled a 12.25 inch diameter hole to a depth of 2010 feet. 9.625 inch diameter, 40.00lbs/ft. casing was set at 2000 feet. The Rotary Kelly Bushing (RKB) is at a height of 20 feet above ground level. All depths are measured from this height. The rig circulated on the hole prior to rigging up the cement head. Schlumberger pumped 735sk of Class A cement, 15.6ppg, 1.23 yield, 2% D178, 0.2% D046, 0.4% B348, 0.3% D202, 0.6% D013. Schlumberger displaced cement with 148bbl of water. Plug was bumped after pumping an additional bbl of water, floats held, 48bbl of cement returned to surface.
