0463016931 | 2018-07-17 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead. Multiple contractors were on location preparing the pad for flow back operations. The production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, two compressors, six sand traps, six gas production units, one vertical vent scrubber, and one combustor. |
0703769520 | 2015-04-30 | Preliminary Restoration | No Violations | On the day of my inspection accompanied by ODNR Inspector Matt Fulk, the site passed preliminary restoration according to ORC 1509.072A. |
1096796873 | 2019-05-01 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead. Production valves were in the open position. Production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, two compressors, six sand traps, six gas production units, one vertical vent scrubber, and one combustor. Identification was located at the well, at the tank battery, and at the entry. Keystone Wireline Swabbing rig was parked on pad. |
1172967501 | 2019-02-27 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead. Production valves were in the open position. Gauges present read 643 pounds per square inch on the 13.375-inch diameter intermediate casing and 680 pounds per square inch on the 9.625-inch diameter intermediate casing. No gauges were present on the 18.625-inch diameter surface casing. The cellar was full of water and no bubbling was present. Production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, two compressors, six sand traps, six gas production units, one vertical vent scrubber, and one combustor. Identification was located at the well, at the tank battery, and at the entry. |
1223523392 | 2018-02-21 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed Patterson UTI Drilling Rig #577 was drilling ahead in the curve with an 8.5 inch diameter bit at 6950 feet with a planned total depth of 18000 feet on 11.7 pound per gallon mud. I met with Ascent Consultant Burl Miller and completed walk around along with reviewing the 9.625 inch diameter casing cement job. During the first stage after switching from the rig pumps to Halliburton returns were lost and were not re-established until the second stage with full returns. Final lift pressure on the first stage was 350 pounds per square inch with full returns on the second stage. Upon reaching total depth and tripping out a Cement Bond Log will be ran and forwarded to ODNR for review. |
1534939536 | 2014-12-06 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed that Highlands Drilling Rig #10 drilled a 23 inch diameter hole to the depth of 484 feet and ran 448 feet of 18.625 inch 87.5#/ft. J-55 casing. Baker Hughes was on site and broke circulation, pumped 35bbls of gel and 10bbls of fresh water. Baker Hughes cemented casing with 575 sacks of Class A lead cement with 0.05% Static Free, 1% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, 0.5% BA-10A, weighing 15.6ppg with a yield of 1.20cuft/sk. Lead cement was followed 179 sacks of Class A tail cement with 0.05% Static Free, 3% CaCl, 0.5% EC-1, 0.125#/sk. Cello Flake, 0.75gals/sk. FP-6L, and 0.5% BA-10A, weighing 15.6ppg with a yield of 1.20cuft/sk. Cement weight was verified by mud scales. Cement was displaced with 123bbls of fresh water. Good cement returns of 73bbls, plug was landed at 302psi and pressured to psi for 5 minutes and the floats at 278psi and pressured to 1028psi for 5 minutes and the floats did not hold pressured up to 1063psi and shut in casing. |
