0049536299 2016-09-15 Production Wells No Violations On the day of my inspection I witnessed the production equipment included 12-400 barrel tanks within containment, 2-NGL tanks, a vapor recovery tower, 2 low pressure horizontal heated separators, a vertical volume pot, 2-oil stabilizer units and a gas combustor all shared with the 1H, 2H, 3H, 5H, 7H, 9H and 10H. Also present for the well was a line heater, a GP unit and a sand trap. The well was equipped with a production tree with the valves in the open position. Gauges present read 1390 pounds per square inch on the tubing and 1670 pounds per square inch on the casing with annular pressure monitoring in place.
0116899739 2015-05-12 Preliminary Restoration No Violations On the day of my inspection I witnessed Schlumberger had completed the cementing of the 5.5 inch production casing circulating 40bbls of Mudpush to surface. Nomac Drilling Rig #72 was preparing to skid the rig to the 34059244500000 HD McClain 28-9-7 9H with re-spud of the 9H later this evening. Petta Enterprises was on site cleaning the pits. The site passed preliminary restoration according to ORC 1509.072A.
0421735880 2015-04-28 Administrative Inspection No Violations I received a call from Nomac Drilling Rig #72 stating they twisted off again with the top of fish at 6520 feet and the bottom at 6753 feet and they were going to set a kick plug from 5395 feet to 4550 feet. I stated they had to cover up the Clinton with cement and Consultant Mathew Whitehead stated he would call their engineer and revise their plug.

Also present for the well was a line heater, a GP unit and a sand trap. The well was equipped with a production tree with the valves in the open position. Gauges present read 245 pounds per square inch on the tubing and 245 pounds per square inch on the casing with annular pressure monitoring in place. The annulus between the 9.625 inch diameter casing and the 5.5 inch diameter casing was at 740 pounds per square inch. I placed call to Chesapeake Regulatory Affairs Advisor Jody Jones and discussed annular pressures. Mr. Jones stated that he is having a meeting later this week with ODNR to discuss annular issues and that he would get back to me with their plan to address. Identification was present and production records were current.

The production equipment included 12-400bbl tanks within containment, 2-NGL tanks, a vapor recovery tower, 2 low pressure horizontal heated separators, a vertical volume pot, an oil stabilizer unit and a gas combustor all shared with the 1H, 2H, 3H, 5H, 7H, 9H and 10H Also present for the well was a GP unit and sand traps planned for installation are currently not on site. The well was equipped with a frac valve in the closed position and drill out is scheduled to begin the week ending 3/26/16.
0925984109 2015-01-11 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Express Energy Services had drilled a 26 inch diameter hole to the depth of 120 feet hitting water at 105 feet and ran 120 feet of 20 inch conductor pipe. Express gained circulation and cemented pipe with 230 sacks Class A cement with 2% CaCl and .5#/sk. of floseal. Displaced cement with 39bbls of fresh water and had good cement returns of 17bbls to surface.

The annulus between the 9.625 inch diameter casing and the 5.5 inch diameter casing was at 840 pounds per square inch.

During my inspection the well was equipped with a plunger lift system with production valves in the open position. Gauges were present on all casing strings. The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 100 pounds per square inch, the 5.5-inch diameter production casing had 400 pounds per square inch, and the 2.875-inch diameter production tubing had 180 pounds per square inch. The production equipment consisted of twelve 400-barrel tanks with one vapor recovery tower, eight sand traps, eight gas production units, two heated horizontal separators, and one combustor. Identification was present and production records were current.
1040273926 2015-04-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #72 had latched onto the fish and broke circulation before twisting off again. Tripped out of the wellbore and calculated top of fish is at 7949 feet. Nomac Drilling Rig #72 will slip and cut their drill line and begin to trip back in the wellbore with an 8.5 inch bit and perform a clean-up run to the top of the fish. Nomac Drilling rig will then trip back out and then trip back in with an overshot to try to latch on fish again.

During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. Annular pressure monitoring was present. The production equipment consisted of twelve 400 barrel tanks with one vapor recovery tower, eight sand traps, eight gas production units, two heated horizontal separators, and one combustor. Identification was present and production records were current. A Chesapeake contractor was on location preforming maintenance on the gas production unit.

Wyoming casing crew was on site running 5.5 inch 23#/ft. production casing at 9580 feet with Schlumberger to cement casing upon completion.
1327787562 2016-03-21 Completion Testing No Violations On the day of my inspection I witnessed Mercer Well Service was onsite drilling out frac plugs at 15110 feet with 3 plugs remaining. Upon completion of drill out MWS will trip out drill string and run 2.875 inch 6.5#/ft. tubing to be set at 8110 feet. Completed walk around with Consultant Buddy Allen revealing proper secondary containment was in place with ongoing production equipment and pipeline installation in progress.
1428413552 2015-04-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #72 was drilling an 8.75 inch diameter hole at a depth of 2973 feet on air with their kick off point at 7295 feet. Intermediate casing was set at 2000 feet and cemented with 720 sacks of class A surface set cement with the plug landing at 21:43 on 4/6/15.
1429797434 2015-04-23 Drill / Deepen / Reopen No Violations On the day of my inspection at 0515 I received a call that Nomac Drilling Rig #72 wanted to set a kick plug after twisting off their BHA on 4/14/15with numerous attempts to completely fish tools from wellbore. Upon arrival for setting of kick plug Nomac Drilling Rig #72 ran 5 inch drill pipe to 7314 feet with the top of fish at 7326 feet and broke circulation. Schlumberger was onsite pumped an 11.5ppg gel pill and Nomac Drilling Rig #72 pulled up to 7214 feet. Schlumberger pumped 30bbls of Mudpush at 14ppg followed by 375 sacks of Class H cement at a weight of 17.5ppg and a yield of .94cuft/sk. Cement was verified with pressurized mud scales and displaced with 98bbls of SOBM. Nomac Drilling Rig #72 tripped 10 stands drill pipe out of cement and broke circulation. Bottom of the fish is at 14500 feet leaving a total of 7286 feet of drill pipe and BHA in wellbore.
1436315067 2015-05-05 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Nomac Drilling Rig #72 was tripping in the hole after replacing their EM device. Nomac Drilling Rig #72 had drilled to 6411 feet before EM failure after beginning their sidetrack at 4900 feet. Nomac Drilling Rig#72 will continue drilling to 7272 feet when they begin their curve with a planned depth of 8267 feet for landing the curve and a planned td of 15859 feet.

Upon inspection Nomac Drilling Rig #72 was tripping in the wellbore with an overshot at 4317 feet with plans to latch on fish and try to break circulation and retrieve fish.

Identification was present and production records were current. A swabbing rig was on location parked near the 1H well.

Gauges present read 3030psi on the tubing and 3330psi on the casing with annular pressure monitoring in place.
1873757143 2015-04-29 Drill / Deepen / Reopen No Violations On the day of my inspection I Nomac Drilling Rig#72 had twisted off while back reaming their initial side kick plug at 6520 feet leaving in the wellbore an 8.5 inch Baker bit, a 6.75 inch Sperry Drill Lobe 6/7 5.0 motor, a X-over sub, a 6.5 x 2.25 inch 99.3# Drill Collar, a X-over sub and 1 stand of 5 inch drill pipe totaling 233 feet. Schlumberger was on site and pumped 2 class H cement plugs from 6407 feet to 5620 feet and 5395 feet to 4550 feet. Nomac Drilling Rig #72 pulled out cement and circulated on wellbore. Nomac Drilling Rig #72 will dress the top of the plug and begin to side track later today.
