The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. |

I met with Ascent Consultant Russ White and discussed the 9.625 inch intermediate cement job as I was notified on 12/24/16 that they had lost returns prior to tripping out of the wellbore. Nomac Drilling Rig #84 had spotted two gel pills prior to tripping out and spotted a third gel pill after landing casing. They gained circulation and Schlumberger successfully cemented the casing with a 2-stage job circulating 46 barrel of cement to surface on the first stage and 43 barrels circulated to surface on the second stage. Ascent will be running a bond log upon completion of the lateral and will supply ODNR with a copy. |

Cement weight was verified by pressurized mud scales. Cement was displaced to 90 feet with 72 barrels of fresh water. Good returns of 30 barrels and cement stayed at the surface. |
