Upon arrival at 17:25 I met with Mr. Carpenter and completed a walk around. Flowback operations had ceased on the 34059244970000 Yost B 5H and the 34059244960000 Yost A 1H along with the 34059244940000 Great Scott 3H and the 34059244950000 Outlaw C 11H being shut-in. The 340592436500 Frank Miller 6H, 34059243670000 Frank Miller 4H and the 34059243660000 Frank Miller 2H remain shut-in with all lines onsite bled down. Inspection of the east corner of the pad revealed a 30 feet by 30 feet area of condensate and brine contamination with a 2 feet by 30 feet area of condensate and brine contamination trailing south along the earthen berm at Global Positioning System coordinates 39.995142 -81.337819 with vacuum trucks onsite removing standing fluids. No off pad impact could be found. Fluids are separated at the Gas Production Unit, piped down a 2 inch steel line, piped into a 4 inch diameter rated for 1500 pounds per square inch flex steel line and then transferred to the Hayes CGF for further breakdown and stabilization. Mr. Carpenter stated that Environmental Management Specialist (EMS) will be performing the clean-up along with Hull & Associates providing sampling and testing with the tentative plan at this time to cut a trough along the area of contamination, continue to pull fluids from trough, remove the flowback frac tanks, begin exposing the lines, determine where the failure is and then remediate accordingly. Owner/Operator shall provide a plan for remediation, remove affected soils, dispose of in a solids landfill, provide disposal manifest to ODNR along with test results confirming that the affected areas have been properly remediated. |

Mix water was tested revealing chlorides of 46 parts per million, pH 7 and 53 degrees. |

Arcadia Construction was now onsite excavating replacing Environmental Management System. Hull and Associates was also onsite performing sampling. Inspection revealed a 30 foot long by 4 feet wide and 5 feet deep trench had been excavated at this point. Eighteen roll-off boxes of contamination being removed to a solids landfill with Semi-trailers being dispatched to assist in hauling due to a backlog of the roll-off boxes. Further inspection revealed that puddles were found near the northeast sump drain line with a visible sheen. No fluid was being released through the mechanical valve. These puddles were field tested revealing a pH of 6 and chloride levels of >6298 parts per million. Hull and Associated performed a field Volatile Organic Compound test revealing positive results for hydro carbons. Eclipse personnel were instructed to place absorbent booms along the rock channel on the northeast side of the pad leading south until the source could be determined and remediated. Excavation will continue until test results provide a negative result for brine and hydrocarbons with confirmatory samples sent to an accredited lab for testing. |

1580303220 | 2017-12-07 | Production Wells | No Violations | On the day of my inspection following up on Compliance Notice 402029661, I witnessed the east sump has been backfilled and collecting water along with the final grade to be completed and all contaminated materials have been removed from site. Water found in the collection sump was field tested revealing pH of 6 and chloride levels of 1895 parts per million up from 1744 parts per million tested on 11/22/17. I met with Eclipse Production Supervisor Brad Paisley and completed walk around. The well was equipped with a plunger lift system with valves in the open position. Gauges present read 1700 pounds per square inch on the tubing and 350 pounds per square inch on the casing with annular pressure monitoring in place and identification present. The production equipment included 2-GP units, 2-line heaters and a sand trap. Also present 3- 400 barrel purge tanks shared with 34059243660000, 34059243670000, 34059243650000, 34059244950000, 34059244970000 & 3405924960000. Fluids and gas are separated at the GP units and then transferred to the Hayes CGF. |

1681836420 | 2017-11-06 | Production Wells | No Violations | On the day of my inspection following up on Compliance Notice 402029661, I witnessed with the recent rainfall the pad was saturated with water, the excavation pit was full along with several areas near the cellars leading to the excavation pit showing a visible sheen. Water from the pad was daylighting above the collection sump which was installed outside the berm last week and was field tested revealing pH of 6 and chloride levels of 492 parts per million. Confirmatory samples from the excavation pit are due back today. Eclipse was instructed with the quantity of water on the pad to retest prior to backfilling if the tests are negative. I met with Eclipse Facilities Foreman Greg Offenberger and discussed the visible sheen present. Mr. Offenberger instructed Arcadia employees to utilize absorbent pads to collect the sheen around the pad. Due to the back log of water trucks it was recommended that frac tanks be dispatched to location, set in containment and pump directly into the tanks. |

I completed walk around confirming at this time containment is being completed for the installation of the onsite produced water tanks. Upon completion tanks will be set and offloading of sand will begin with commencement of frac to follow. |

During the meeting progress of remediation, continued remediation and a summary report were discussed. Upon completion of meeting Eclipse personnel conducted a tour of the production equipment on pad and how it relates to the Hayes CGF. Upon completion due to high traffic and construction at the Hayes CGF, the tour continued at the Borton CGF. Michael Barker of Enlink instructed how gas, water, condensate and natural gas liquids flow into the facility, through the facility and then out of the facility through their equipment. |
