0038550672 2017-07-11 Casing String Witnessed No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 had drilled a 17.5 inch diameter hole to the depth of 1572 feet. Wyoming Casing Crew ran 228.21 feet of 13.375 inch 61#/ft. J-55 casing and 1293.97 feet of 13.375 inch 54.5#/ft. J-55 casing for a total of 1522.18 feet of 13.375 inch casing set at 3.2 feet above KB and washing down the last 7 joints. KB height is 30 feet above ground level. Schlumberger was on site and loaded their lines with 5 barrels of fresh water and performed a low pressure test at 900 pounds per square inch and a high pressure test at 3300 pounds per square inch. Schlumberger gained circulation with 260 barrels of fresh water, 20 barrels of gel and 5 barrels of fresh water. Schlumberger cemented casing with 1205 sacks of Class A cement 15.6 pounds per gallon, 1.21 cubic foot per sack cement with 3% D154 Extender, .2% D013 Retarder, .25 pound per sack D130 Flake, .3% B160A Dispersant and .4% B348 FLAC. Cement weight was verified by pressurized mud scales and displaced with 226 barrels of fresh water. Cement was circulated to surface with good returns of 46 barrels, plug was landed at 580 pounds per square inch and pressured to 1157 pounds per square inch for 5 minutes and the floats held with 1 1/2 barrels back. Mix water was tested revealing chlorides of 87 parts per million, pH 7 and 68 degrees.
0195685069 2017-07-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 had reached total depth in their later at 24060 feet, circulated the wellbore and tripped drill pipe out. Wyoming Casing crew had just completed rig up, safety meeting and picking up 5.5 inch diameter 23 pound per foot P-110 casing to be set at 24034 feet KB with cementing to follow.

Location prep continues with the air package set and rig containment being laid as H&P Drilling Rig #531 will begin to move later this week.
0438758159 2018-07-20 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. Gauges were present reading 1020 pounds per square inch on the production tubing,1650 pounds per square inch on the production casing, 300 pounds per square inch on the 9.875-inch diameter intermediate, and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of six indirect line heaters, nine sand traps, nine gas production units, and three 400-barrel tanks. Well product travels via pipeline to the Hayes Central Gathering Facility.
0691448918 2018-02-26 Completion Testing No Violations On the day of my inspection accompanied by ODNR Inspector Ryan Armstrong, I witnessed the well was flowing back with through Energes flowback equipment to sales with 3300 pounds per square inch on the tubing and 3500 pounds per square inch on the casing at 3.3 MMCF per day and 40 barrels per hour of oil and 0 barrels per hour of water through a 20/64 inch choke. We met with Consultant Cristo Giannareas and completed walk around secondary containment was in place.
1152088050 2018-06-11 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. The well was equipped with a production tree style wellhead with a plunger lift system. Production valves were in the open position. Pressure monitoring for all casing strings were present. The production equipment consisted of three 400 barrel tanks, nine sand traps, nine gas production units, and six indirect line heaters. The identification was located at the wellhead. The area effected by the pad construction did not have any erosion present and vegetation was established. The pad surface consisted of gravel with a fence surrounding the perimeter. The location meets preliminary restoration requirements written under 1509.072 (A).
1496249331 2017-05-27 Casing String Witnessed No Violations The purpose of my inspection was to witness 120 feet of the 24 inch diameter conductor being cemented in a 30 inch diameter hole that is 120 feet in depth. Precision Rathole drilled the conductor with their rotary rig and auger bit. Water was encountered at 50 feet in depth. During my inspection Stingray Cementing was on location to complete the cement operations. Job calculations were discussed with Stingray and Eclipse Company Man. A pre job safety meeting was held with all personnel on location. I witnessed Stingray pump 10 barrels of fresh water, 62.5 barrels of class A cement, displaced with 55 barrels of fresh water, and 15 barrels of cement returns were established in the cellar.
1500991551 2017-07-17 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 had drilled with an 8.75 inch bit to the bottom of the curve at 8290 feet on 11.7 pound per gallon mud and was tripping out of the wellbore at 4219 feet. H&P Flex Rig #531 will pick up the Schlumberger 8.5 inch diameter orbit tool and trip back in to drill the lateral to a planned total depth of 24031 feet.
1512658372 2017-12-07 Completion Testing No Violations On the day of my inspection I witnessed Halliburton Services complete stage 15 of 116 with a breakdown pressure of 6751 pounds per square inch, an average treating pressure of 9766 pounds per square inch, an average rate of 82.4 barrels per minute and a total of 260740 pounds of proppant pumped. I completed walk around with Eclipse Consultant Diamon Girod revealing sand box containers are being utilized and the sand is offloaded from the boxes sent via a belt system to the blenders.
1533970854 2017-07-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 was preparing to trip in the wellbore and re-spud well with 17.5 inch bit and drill the surface section to a planned depth of 1554 feet. Upon completion 13.375 inch diameter casing will run and cemented.
