0322357846 2019-02-07 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges were present on all casing strings. The 13.375-inch diameter surface casing had 100 pounds per square inch and the 9.625-inch diameter intermediate had 720 pounds per square inch. The production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Ascent well tender was on location along with two Wise tanker trucks that were loading produce fluids from the tank battery.
0619163798 2017-07-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 (formerly Nomac Drilling Rig #84) drilling ahead with a 12.25 inch diameter bit on air at 1840 feet with a planned total depth for this section of 6700 feet with 9.625 inch diameter L-80-40 pound per foot casing to follow. I met and completed walk around with Ascent Consultant Alan Rice.
0952737510 2017-05-17 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Drilling Rig #26 had drilled a 17.5 inch diameter hole to the depth of 1225 feet and Wyoming casing crew ran 1213.62 feet of 13.375 inch 61 pounds per foot J-55 casing 4.6 feet above KB. O-Tex Pumping LLC was on site and gained circulation with 5 barrels of fresh water, 35 barrels of gel, 10 barrels of fresh water and cemented casing with 1340 sacks of Class H cement with 1.5% Calcium Chloride, .2% X--<39 parts per million, pH 7 and 60 degrees.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of nine 400 barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.

1500299539 2017-07-17 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 was back to drilling ahead in their lateral after two failed mud motors with a 8.75 inch diameter bit on 11.7 pound per gallon mud at 10123 feet with a planned total depth of 19850 feet. I met with Ascent Consultant Alan Rice who notified me earlier today that they were flaring and during the inspection they were experiencing 2341 units of gas with an intermittent 1 to 3 foot flare.
1500908346 2017-07-24 Casing String Witnessed No Violations On the day of my inspection I witnessed that after Patterson UTI Drilling Rig #584 had reached a total depth of 18999 feet, 5.5 inch diameter 23 pound per foot P-110 casing was set at 19898.5 feet set at 3.5 feet above KB. KB height is 25 feet above ground level. Schlumberger was onsite and pumping the last 43 barrels of tail cement. Tail cement was 4100 sacks of Class H weighing 15.6 pounds per gallon and a yield of 1.03 cubic foot per sack. Cement weights were verified by pressurized mud scales throughout the job. Plug was dropped and cement was displaced with 420 barrels of water. Plug was landed at 2480 pounds per square inch and pressured up to 3250 pounds per square inch for five minutes and the floats held with 4 barrels back.

The production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and two combustors. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1694478795 2017-05-02 Casing String Witnessed No Violations On the day inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet, hitting water at 40 feet and ran 100 feet of 30 inch 118.65 pounds per foot conductor pipe. O-Tex Pumping LLC was onsite and pumped 2 barrels of fresh water, performed a 150 pounds per square inch pressure test, gained circulation with 8 barrels of fresh water and cemented pipe with 330 sacks Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 90 feet with 74.8 barrels of fresh water with good cement returns of 15 barrels to surface. Mix water was tested revealing chlorides of 54 parts per million, pH 7 and 59 degrees.
