0476904629 2017-08-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 was drilling ahead in the lateral with an 8.5 inch diameter on 11.9 pound per gallon mud at 18025 feet with a planned total depth of 20062 feet. I completed walk around with Ascent Government Relations Manager Amanda Finn, Ascent Drilling Superintendent Richard Thompson and Ascent Consultant Alan Rice.
0554824168 2019-02-07 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges were present on all casing strings. The 13.375-inch diameter surface casing had 110 pounds per square inch and the 9.625-inch diameter intermediate had 860 pounds per square inch. The production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Ascent well tender was on location along with two Wise tanker trucks that were loading produce fluids from the tank battery.
0913220272 2017-05-02 Casing String Witnessed No Violations On the day inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet, hitting water at 40 feet and ran 100 feet of 30 inch 118.65 pounds per foot conductor pipe. O-Tex Pumping LLC was onsite and performed a 150 pounds per square inch pressure test, gained circulation with 2 barrels of fresh water and cemented pipe with 330 sacks Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 90 feet with 74.8 barrels of fresh water with good cement returns of 25 barrels to surface. Mix water was tested revealing chlorides of 54 parts per million, pH 7 and 59 degrees.

The production equipment consisted of nine barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor.
1344323323 2017-08-03 Drill / Deepen / Reopen No Violations On the day of my inspection following up from call received last night concerning Patterson UTI Drilling Rig #584 losing 78 feet of their Bottom Hole Assembly (BHA) 70 degrees in the curve at 7884 feet leaving the top of the fish at 7806. Upon arrival I met with Ascent Drilling Superintendent Richard Thompson and reviewed the event. Currently UTI Drilling Rig #584 was tripping out of the wellbore at 6500 feet after tripping in with a KLX 7.875 inch Cutlip Overshot with a 6.375 inch grapple and successfully retrieving feet of the fish. Patterson UTI Drilling Rig #584 will make up a new BHA and trip back in and begin completing the curve.
1495361692 2017-05-21 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with Falcon Drilling rig 20 and O-Tex cement service. The rig has drilled a 17.5 surface borehole to a measured depth of 1,225 and installed 13,375 diameter J-55 grade buttress thread casing to a depth of 1,208. A casing shoe and float collar were ran and 10 centralizers were installed on the casing. The rig loaded the well bore with fresh water and circulated the last casing joint into the well bore. O-Tex Cement Service cemented the casing using a 284 barrel cement mix consisting of 1,325 sacks of Class H cement. The cement weight was verified at 15.6 pound per gallon by weight of scale. 35 barrels of 6% gel spacer was ran ahead of the cement. The cement was displaced to the float collar set at 1,163 with 177 barrels of fresh water and the casing was pressure tested to 1,150 (psi) pound per square inch upon the landing of the cement plug. The final lift pressure prior to landing the plug was 610 psi. 74 barrels of cement returns were witnessed to surface inside the cellar.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of nine 400 barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
