0368762573 | 2018-10-16 | Preliminary Restoration | No Violations | The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of three 400-barrel tanks on the West side of the pad, eight horizontal line heaters, eight sand traps, and eight gas production units all on the South side of the pad. The pipe line tie in was located on the North side of the pad. Location meets requirements for preliminary restoration. |
0378211020 | 2018-07-19 | Preliminary Restoration | No Violations | The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure gauges for all casing strings were present and in plain view with a casing identification tag. The production equipment consisted of three 400-barrel tanks on the West side of the pad, eight horizontal line heaters, eight sand traps, and eight gas production units all on the South side of the pad. The pipe line tie in was located on the North side of the pad. Eight 500-barrel Frac tanks are located on the West side of the pad. Due to signs of erosion on the cut slope on the West side of the pad the location does not meet Preliminary restoration requirements written under ORC 1509.072 (A). I will consult ODNR Engineering section and discuss the cut slope erosion. |
0414519711 | 2017-08-16 | Casing String Witnessed | No Violations | On the day inspection I witnessed that Precision Rathole had drilled a 30 inch diameter hole to the depth of 117 feet, hitting water at 17 feet and ran 120 feet of 24 inch diameter 94.62 pounds per foot conductor pipe with a 3 foot stick up. Due to a plug job, ODNR Inspector Austin Gutridge relieved me to witness the cement job. |
0678917671 | 2017-09-24 | Casing String Witnessed | No Violations | On the day of my inspection I witnessed that H&P Flex Rig #531 had drilled a 17.5 inch diameter hole to the depth of 1361 feet. Wyoming Casing Crew ran 221.39 feet of 13.375 inch 61 pound per foot J-55 casing and 1093 feet of 13.375 inch 54.5#/ft. J-55 casing for a total of 1314.39 feet of 13.375 inch casing set at 3.4 feet above KB. KB height is 30 feet above ground level. Schlumberger was on site and loaded their lines with 5 barrels of fresh water and performed a low pressure test at 1000 pounds per square inch and a high pressure test at 3000 pounds per square inch. Schlumberger gained circulation with 200 barrels of fresh water, 20 barrels of gel and 10 barrels of fresh water. Schlumberger cemented casing with 1060 sacks of Class A cement 15.6 pounds per gallon, 1.21 cubic foot per sack cement with 3% D154 Extender, .2% D013 Retarder, .020 gallon/sack D047 Antifoam, .25 pound per sack D130 Flake, .3% B160A Dispersant and .4% B348 FLAC. Cement weight was verified by pressurized mud scales and displaced with 195.3 barrels of fresh water. Cement was circulated to surface with good returns of 33 barrels, plug was landed at 550 pounds per square inch and pressured to 1150 pounds per square inch for 5 minutes and the floats held with 1 barrel back. Mix water was tested revealing chlorides of 180 parts per million, pH 7 and 65 degrees. |
1000443606 | 2017-10-02 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed that H&P Flex Rig #531 had landed their curve at 8105 feet and was drilling ahead on 11.8 pound per gallon mud in the lateral with a 8.5 inch diameter bit at 9830 feet with a planned total depth of 28764 feet. I met with Eclipse Consultant Jack Turner and completed a walk around revealing 5.5 inch diameter 23 pound per foot casing was being offloaded. |
1482906227 | 2019-03-27 | Production Wells | No Violations | The purpose of my inspection was a status check. On the west side of the pad the top of the high wall on the cut slope was eroded toppling material sedimentation down near the tank battery. Mark Atkinson with ODNR Engineering was aware of the issue and he has been in contact with Eclipse Resources. |
1712270709 | 2018-08-07 | Preliminary Restoration | No Violations | The purpose of my inspection was a status check on a preliminary restoration. The pipe line tie in was located on the North side of the pad. Three 500-barrel Frac tanks are located on the West side of the pad. Signs of erosion remain on the cut slope on the West side of the pad. I spoke with Mark Atkinson with the ODNR Engineering Section and he stated that they would investigate the situation. |
1761581080 | 2017-10-10 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I witnessed that H&P Flex Rig #531 had reached a total depth of 28775 feet and Wyoming Casing Crew was onsite running 5.5 inch diameter 23 pound per foot production casing at 25360 feet with cementing to follow. I completed walk around with Eclipse Consultant Neil Ingram. |
1924689262 | 2017-08-16 | Casing String Witnessed | No Violations | The purpose of my inspection was to witness 120 feet of 24 inch diameter conductor being cemented in a 30 inch diameter borehole. Water was encountered at 17 feet in the drilling process. Stingray Pressure Pumping was onsite and pumped 10 barrels of fresh water, 290 sacks Class A cement with 2% calcium chloride, and 75 barrels of fresh water for displacement. 20 barrels of cement returns were witnessed in cellar. |
