0092038525 2019-03-27 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of three 400-barrel tanks on the West side of the pad, eight horizontal line heaters, eight sand traps, and eight gas production units all on the South side of the pad. The pipe line tie in was located on the North side of the pad. On the west side of the pad the top of the high wall on the cut slope was eroded toppling material sedimentation down near the tank battery. Mark Atkinson with ODNR Engineering was aware of the issue and he has been in contact with Eclipse Resources.
0598936850 2018-07-19 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure gauges for all casing strings were present and in plain view with a casing identification tag. The production equipment consisted of three 400-barrel tanks on the West side of the pad, eight horizontal line heaters, eight sand traps, and eight gas production units all on the South side of the pad. The pipe line tie in was located on the North side of the pad. Eight 500-barrel Frac tanks are located on the West side of the pad. Due to signs of erosion on the cut slope on the West side of the pad the location does not meet Preliminary restoration requirements written under ORC 1509.072 (A). I will consult ODNR Engineering section and discuss the cut slope erosion.
0987760629 2017-09-12 Drill / Deepen / Reopen No Violations On the day of my inspection, I accompanied ODNR Inspector Doug Bennett and witnessed H&P Flex Rig #531 had reached total depth at 7170 feet and tripped drill pipe from wellbore. Wyoming Casing Crew was onsite rigging up to run 9.625 inch diameter 40 pound per foot P-110 casing to be set at 7107 feet. A DV tool will be set at 1707 feet with a 2-stage cement job to follow. We met with Eclipse Consultant Neil Ingram and completed a walk around.
1013916252 2018-03-12 Completion Testing No Violations On the day of my inspection I witnessed Nine Wireline was onsite to plug and perforate stage 52 of 84. S3 Pumping pumped tools down with 254 barrels of produced water at 17 barrels per minute with 7200 pounds per square inch. Nine Wireline successfully set the plug at 13525 feet and perforated the casing with 36 shots from 13506 feet to 13375 feet. I met with Eclipse Consultants Jimmy Metcalf, Lance Crumbling and Cristo Giannareas and completed walk around. Frac operations were suspended as Halliburton was making repairs to their missile.
1359593387 2018-08-07 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure gauges for all casing strings were present and in plain view with a casing identification tag. The production equipment consisted of three 400-barrel tanks on the West side of the pad, eight horizontal line heaters, eight sand traps, and eight gas production units all on the South side of the pad. The pipe line tie in was located on the North side of the pad. Three 500-barrel Frac tanks are located on the West side of the pad. Signs of erosion remain on the cut slope on the West side of the pad. I spoke with Mark Atkinson with the ODNR Engineering Section and he stated that they would investigate the situation.
1503152342 2017-08-16 Casing String Witnessed No Violations On the day inspection I witnessed that Precision Rathole had drilled a 30 inch diameter hole to the depth of 119 feet, hitting water at 17 feet and ran 120 feet of 24 inch diameter 94.62 pounds per foot conductor pipe with a 1 foot stick up. Stingray Pressure Pumping was onsite and pumped 2 barrels of fresh water, performed a 500 pounds per square inch pressure test and cemented pipe with 290 sacks Class A cement with 2% CaCl and .25 pound per sack of Celluseal weighing 15.6 pounds per gallon and a yield of 1.21 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 109 feet with 57.75 barrels of fresh water with good cement returns of 10 barrels to surface. Mix water was tested revealing chlorides of 46 parts per million, pH 7 and 65 degrees.

1534158836 2017-09-20 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 had reached total depth of 22015 feet, circulating the wellbore with 12.5 pound per gallon mud and tripping out of the wellbore at 14180 feet. Upon completion Wyoming Casing Crew will run 21994 feet of 5.5 inch diameter production casing and upon completion Schlumberger will cement the casing. I completed walk around with Eclipse Consultant Jack Turner and reviewed the production casing cement proposal with Eclipse Cementing Supervisor Nathan Anderle.
