0647312081 2019-05-01 Completion Testing No Violations The purpose of my inspection was a status check on completion operations. During my inspection Halliburton was on location completing hydraulic stimulation operations. Schlumberger Wireline was rigged up and tripping in hole to set the frac plug for stage 59 of 64.
0912823020 2018-12-28 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a work-over gate valve. The 13.375-inch diameter surface casing had 200 pounds per square inch, the 9.625-inch diameter intermediate casing had 0 pounds per square inch, and the 5.5-inch diameter production casing had 0 pounds per square inch. The 5.5-inch diameter casing was equipped with Sage Rider Subsurface Surveillance System providing real-time downhole pressure and temperature data. The well was awaiting hydraulic stimulation operations. The location meets preliminary restoration requirements written within the Ohio Revised Code 1509.072 (A).
1124529491 2019-01-17 Completion Testing No Violations The inspection revealed that the frac- head is on the well and is under toe prep operations. The wells have heat being blown back in the cellars, the base of the well head is covered up. The frac is scheduled for 2-26-19.
1384582197 2018-08-09 Casing String Witnessed No Violations The purpose of my inspection was to witness the cementing of the conductor. The conductor was 30-inches in diameter and weighed 118.65 pound per foot. Petroset was on location with 1 bulk cement truck and one pump truck. The borehole was 36 inches in diameter and was drilled to 100 feet in depth. Water was encountered during the drilling process at 70 feet. I witnessed Butchs rathole weld and run the 30-inch diameter conductor to 100 feet. Petroset plumbed up to a 2-inch diameter threaded nipple that was welded on the top plate of the conductor and pumped 315 sacks of class A cement weighing 15.6 pounds per gallon and 75 barrels of fresh water for displacement. 1 foot of cement returns were used to seal the cellar floor. The cellar was 10 feet in diameter and 8 feet deep.
1540901666 2018-10-29 Drill / Deepen / Reopen No Violations During my inspection Pioneer 77 was experiencing some electrical issues and they were not in operation. They have the 8.5-inch diameter production borehole drilled to 15901 feet. The kick of point at 6450 feet, the landing point at 7195 feet, and the shoe of the 9.625-inch diameter intermediate casing was set at 6384 feet. I was told by the Ascent company man Mark Smith that the borehole was drilled to 6358 feet and a formation integrity test was completed to resemble drilling with 15 pound per gallon mud equaling 1487 pounds per square inch.
1541770249 2018-11-02 Casing String Witnessed No Violations Pioneer 77 had the 8.5-inch diameter production borehole at the total measured depth of 17764 feet. During my inspection ODNR Southeast Regional Supervisor Brent Bear and I witnessed Franks International along with Sage Rider running the 5.5-inch diameter production casing. Sage Rider installed 2 cables on the outside of the 5.5-inch production casing from surface to total depth. One cable was a fiber optic cable to measure acoustic signals and the second cable was to gather data from the pressure gauges located within the 5.5-inch casing. The pressure gauges within the casing will not be in operation until prior to hydraulic fracturing completions when the casing is pressure up on to activate gauges.
1599060918 2019-03-04 Completion Testing No Violations The purpose of my inspection was a status check. During my inspection the location was being prepared for hydraulic stimulation operations. The well was equipped with a frac tree. An aboveground water storage tank had been assembled on the Northeast corner of the pad.

1822208954 2018-08-16 Casing String Witnessed No Violations The purpose of my inspection was to witness the cementing of the surface casing. 13.35-inch diameter surface casing was ran to 1050 feet within the 17.5 inch diameter borehole. During my inspection I witnessed C&J Services O-Tex Cementing rig up on location. A safety meeting was held with all personnel on location. Following the safety meeting O-Tex pumped 35 barrels of gel, 10 barrels of fresh water, 1105 sacks (238 barrels) of class A cement, and 154 barrels for displacement. The cement was weighed in pressurized mud scales confirming the weight of 15.6 pounds per gallon. Cement returns established.
