The pad was constructed with disturbed areas restored in the year 2014. In the year 2019 two additional wells were added to the pad. The cuttings from the two cellars and two conductor boreholes were stockpiled on the South side of the pad with vegetation established. The pad slopes appeared to be stable. The well was being prepared for flowback and the wellhead was being assembled. The production equipment was being hauled on to location and arranged on pad. Identification signs and drilling permits were posted at entry. The location meets the requirements for preliminary restoration as per the Ohio Revised Code 1509.072 (A). |

During my inspection Patterson 584 was drilling at 11,338-feet within the lateral section of the production borehole. The kick off point before the curved section was at 6864-feet and the landing point after the curved section was at 8185-feet. On 3/15/19 the 40 pounds per foot 9.625-inch diameter intermediate casing was run and cemented at 6750-feet in two stages to surface. The post cement job was reviewed with the Ascent company man, Michael Burton and 40 barrels of cement returns were established. The 5.5-inch diameter production casing was being hauled onto location and placed on the pipe racks. |

Following the safety meeting Schlumberger pumped 25 barrels of gel, 10 barrels of fresh water, 1372 sacks of class A cement, and 200 barrels for displacement. The cement was weighed in pressurized mud scales confirming the weight of 15.6 pounds per gallon. Estimated 65 barrels of cement returns established and weighed at 15.2 pounds per gallon |
