0437320531 2015-12-04 Production Wells No Violations On the day of my inspection I received a phone call from Alliance Resources Supervisor Bruce Close stating that he would like some direction on the email he received from Carrizo on maintenance issues that needed addressed at the well location. Upon arrival I witnessed the production equipment included a 100bbl tank within an earthen dike, a horizontal separator, a 2 inch sales meter with a 1 inch orifice and a chart recorder within a separate dike. The well was equipped with 8.625 inch casing, 4.5 inch casing, 1.5 inch tubing and a plunger lift system with valves open on the tubing and 1 of 2 open on the casing supplying the motor valve. Pressure gauges present read 170psi on the tubing and 220psi on the casing. Identification was present and production records were current. Inspection further revealed an old tank ladder laying along the side of the dike, a very small pin hole in the 100bbl tank at 2.5 feet from bottom with minor seepage and minor erosion on the access road. I placed call to Mr. Close and stated that I recommended the ladder be removed along with dressing the access road to be compliant with ORC 1509.072A and also to keep an eye on the tank as it appears to have a very small pin hole which could result in possible further compliance issues if seepage would reach the dike.
0586082088 2016-03-23 Production Wells No Violations On the day of my inspection following up on Complaint Number 5284, a complaint where an adjourning land owner was concerned about what he thought to be a possible brine leach that was coming from the from the dike and leaching outward into his property. Upon my arrival I witnessed a pinhole leak coming from the tank battery; inside the dike there was evidence that brine was present around the tank battery and separator area. After the inspection of the dike, I noticed an area that measured approximately 30 feet by 60 feet on the lower side of the dike that was vegetation lacked and showed signs of brine contamination, coming from the 30 feet by 60 feet on the lower east side was an area that measure approximately 15 feet by 40 feet that also showed signs on contamination. The source of the brine concentration outside the dike appeared to be coming from a 2 inch drain pipe coming from the dike and dripping outside of containment. To insure that this was a brine spill, I performed a field test using Chloride strips and found brine concentrations of approximately 750 ppm. After the conduction of my inspection, I placed a call to Matt Postal Weight (An Alliance Petroleum Supervisor) and informed him of the issues I had discovered. Matt stated he was unaware of the situation and asked if we could meet on location to address the issues. Since I was unavailable to meet on the next day, Matt agreed to meet with Doug Bennett to perform a site over and make a plan to remedy the issues.
1106616686 2016-04-19 Production Wells No Violations On the day of my inspection following up on Complaint Number 5284, I witnessed the spill is fully remediated and vegetation growth is re-established. While conducting my inspection I walked the entire effected spill area and did not see any signs of pollution of contamination.

-up operation. On the day of my inspection I witnessed Sutton Pump Supply was in the process of replacing the tank battery and flow line. All contaminated soils had been removed and new soils were now in place. The new soil was seeded, limed and covered with straw to promote grass growth. After the tank is replaced and the flow line is fixed Matt plans to have the well back into production.

2012720746 2016-03-24 Production Wells No Violations On the day of my inspection following up on Complaint ID 5284, I met with Alliance Petroleum Corporation Production Foreman Matt Postlethwait. Field testing was completed at GPS coordinates 40.193929 -81.339521 revealing chlorides of 1082ppm and a pH of 5. Further field testing was completed at GPS coordinates 40.193868 -81.337785 revealing chlorides of 108ppm and a pH of 6. Mr. Postlethwait shut the well in and was instructed to pull the fluids from the tank, determine whether the flowline or salesline was leaking, repair lines, remove tank, remove contaminated soil, replace with clean fill, rebuild dike, dispose contaminated soil in a solids landfill and provide disposal manifest to ODNR. Mr. Postlethwait stated the tank would be emptied today and remediation would begin tomorrow if they receive landfill approval.
