0239781001 2016-04-14 Plug / Plug Back No Violations On the day of my inspection accompanied by Inspector Mark Moreland, I witnessed Atlas Noble has a crew on site working on the final restoration process and the well head has been cut 3 feet below surface. While on site I met with Company Supervisor Chuck Woods, who informed me that the restoration process will be completed when the water saturated grounds dry up and allow for better conditions to grade the grounds. I will be back on site at a later date to complete the final restoration.
0469645076 2016-05-31 Plug / Plug Back No Violations On the day of my inspection accompanied by inspector Mark Moreland, I witnessed the site could not pass final restoration due to the fact the location has yet to be graded and seeded. After the inspection I spoke to Chuck Woods who informed me that we would have a service company reclaim the location within the next two days.
1140818261 2016-04-01 Production Wells No Violations On the day of my inspection accompanied by Inspector Mark Moreland following up on NOV 844929800, I witnessed the well consist of 4.5 inch diameter production casing with a 4.5 inch diameter to 2 inch diameter changeover at the top of the production casing going into the production flow-line. The valve leading into the flow line is in the closed position indicating that the well is idle at the time of my inspection. Production equipment for the well still includes a 210bbl. Production tank, 30bbl. Steel brine tank and a horizontal separator with a 2 inch diameter meter run inside of the unit. Since my last inspection, I witnessed the issue of H2S gas leak that was said to be repaired 7/30/2015, is still occurring. The Honeywell PhD6 gas detector picked up a reading of 10 ppm of H2S at the well head. Due to the reading, the inspection was shut down and we left the area. After reaching a safe distance from the well head, Mark then placed a call to Pat Ervin. Mark stated to Pat that the well is still leaking H2S and needs to be resolved immediately. Pat then stated to Mark that he was aware of the issue and he was working on the process to get the well plugged as soon as possible. This is a lower laying area where it is easy for the gas to dwell and accumulate; there is a house approximately 200 feet from the well bore. A definite safety issue should be noted and the well needs to be plugged immediately.

1438877267 2015-07-30 Production Wells No Violations This well had an H2S leak which has been repaired. There are no leaks on the well today and the well appears to be back in operation. The well has not been repaired just the leak on the 2 inch line. The well will need repaired and or plugged. The warning sign should remain at this well head to advise everyone of the danger of possible H2S gas leaks.
1460379161 2016-04-11 Plug / Plug Back No Violations Nicks Well Service has 2-3/8 inch diameter tubing at 5687 ft. and set the first plug today from 5687 ft. to 5400 ft. cemented with 25 sacks of class A cement 1.18 in weight. Nicks Well Service is providing the service rig, cement trucks, water truck and tubing trailer. Appalachian Well Service then shot the 4-1/2 inch diameter casing at 4800 ft. Nicks Well set the 2nd plug at 4800 ft. to 4600 ft. cemented with 57 sacks of class A cement at 1.18 - They did not reverse circulate and the cement did not drop out of the 2-3/8 inch tubing. Appalachian Well Service could not get shot down the tubing. Nicks Well tried to pump on the tubing and it pressured up to 1000 psi. Tubing then had to be pulled wet and another string used to cement. After reviewing the cement info I advised that they could rip the 4-1/2 inch casing and use it to finish the plug operation because I could prove casing integrity. Nicks will now pull all of the tubing out and rip the casing then pack off the well and continue the operation on Tuesday.
1460551710 2016-04-13 Plug / Plug Back No Violations Nicks Well Plugging has ripped the 4-1/2 inch diameter casing for the 2nd time at 1565 ft. The first rip was at 4210 ft. Due to the H2S gas readings on Tuesday the crew had to shut the well in after setting plug # 3- That was to allow the H2S gas to subside. Readings during the displacement went above 270 PPM -Today while setting plug # 4 readings went to 17 PPM for a brief time and subsided to below 12 PPM- Plug # 4 was set at 1446 to 1246 ft. with 60 sacks of class A cement at 15.6 lbs. Plug # 5 was set at 200 to 0 ft. with 100 sacks of class A cement at 15.6 lbs. Circulated cement to surface where it remained after the job was complete. Advise Atlas to pump off pits ASAP and notify me when they plan to cut off the casing.
1461240460 2016-04-21 Plug / Plug Back No Violations I am on site to check this well for leaks or signs of leaks. The area is rough graded and will need graded and seeded. There is a dike at the road which will need removed along with the remaining production equipment. This well has just been plugged and the ground is not dry enough to complete the final graded.
1579244903 2015-04-10 Production Wells No Violations I am checking this well after receiving a call from Pat Irvin of Atlas Noble to inform me that it has been repaired and all of the leaks have been fixed. I arrived to find that I could not hear or smell any leaks or H2S gas today. There is a new 4 to 2 change over on the well head with another valve in place. The roads in and out of the area are closed due to flooding.
1756539382 2016-01-12 Production Wells No Violations On the day of my inspection following up on NOV 844929800, I witnessed the well consist of 4.5 inch diameter production casing with a 4.5 inch diameter to 2 inch diameter changeover at the top of the production casing going into the production flow-line. Production equipment for the well includes a 210bbl. production tank, 30bbl. steel brine tank and a horizontal separator with a 2 inch diameter meter run inside of the unit. The production equipment is placed inside an earthen dike that measures 20*60 at an average height of 2 feet. The issue of H2S gas leak was said to be repaired 7/30/2015, however the leak has reoccurred. The Honeywell PhD6 gas detector picked up a reading of 10 ppm of H2S at the well head. Due to the reading, the inspection was shut down and I left the area. This is a lower laying area where it is easy for the gas to dwell and accumulate; there is a house approximately 200 feet from the well bore. A definite safety issue should be noted.
2041233306 2016-04-12 Plug / Plug Back No Violations Nicks Well Service is on site today to continue the plugging operation. Specialized Safety Service is on site again to monitor for H2S. Nicks Well has pulled out all of the 2-3/8 inch diameter tubing and ripped the 4-1/2 inch diameter casing at 4210 ft. Plugging could not start because the pits are full and the tubing trailer will need removed before the next plug is started. There is a water truck on site to pump off the pit and another trailer will be backed into place for the 4-1/2 inch casing. Set plug # 3 at 4170 ft. to 3970 ft. cemented with 57 sacks of class A cement at 15.6 lbs. After plug was set fluid circulated out H2S and on two occasions everyone was moved off site to a free area until H2S subsided. They had problems with the balance of the plug the well was shut in before casing could be pulled. The plan now is to leave the plug set up and return Weds. They will need to rip the 4-1/2 inch casing again and then pull up to set the remaining 2 plugs. Pit water has H2S dissolved in it and is de- gassing. It may not be safe to pull the fluid today until it has de-gassed. Reading went above 280 PPM I advised that the people in the home should remain out of the home until after the well has been plugged.
