1058189649 2016-06-28 Production Wells Violations Noted I am checking older wells in this area due to recent H2S leaks. I discovered that the 110 bbl. Production tank is leaking at the weld. There are salt crystals forming on the weld near the bottom of the tank and it is damp under the tank at the same location. The tank identification is poor and needs to be replaced along with the tank. I contacted Brian Tipka and advised of the tank leak and lack of identification. He told me that the tanks are to be removed and the area under the tank cleaned up during that time. He is to begin within the next week if weather permits. While on location the pumper arrived and I went over the phone call information with him.
1261305187 2017-10-30 Production Wells No Violations On the day of my inspection I am following up two compliance notices. In April 2006 Inspector Farnsworth wrote a failure to keep dike free of oil and in June 2016 Inspector Moreland wrote well causing pollution and contaminaton for a leaking oil tank. NOV # 1389805923 from Farnsworth and CA# -1058189649 from Moreland.

Onsite today, the inside of containment is free of oil and water and the oil tank has been replaced with a competent tank. Well is equipped with a 4.5 inch diameter casing and 2.375 inch diameter changeover on top. Well is shut in. Containment holds a 110 barrel steel tank, horizontal seperator with meter box, and a domestic meter. Chart is still being changed. Inspection report has been dupolicated to address both NOV/CA numbers.

1360358200 2006-04-13 Production Wells Violations Noted Routine inspection revealed that there is an area (app. 12 x 40) of pollution and contamination (black oil stained area) inside the tank battery SPCC dike . The well head consists of 4 1/2 casing and a 4 1/2 to 2 swege tied into the flowline. The tank battery consists of two 100 bbl oil storage tanks and one 50 bbl brine storage tank .
1389805923 2014-01-14 Production Wells No Violations I am on site today to check this well to see if the clean up of the inside of the dike has been completed. I found that it has not and have contacted the operator at 330-339-4411 and advised them that a clean up is needed.
1395680600 2014-03-24 Production Wells No Violations I am checking this site to see if there has been any clean up inside the dike area. I found that no work has been done and that the contamination inside the dike remains. Next I contacted Brian Tipka @ 330-339-4411 and ask why he had not returned my call. He said that he was busy and that he was on another clean up in Tuscarawas Co. I ask why the site had not been cleaned up to which he said that he was busy. I explained that this well had a violation from 4/17/2006 and that no more time would be given for the clean up that 7 years was long enough. They are to have the site clean in the next two weeks.
1450897628 2015-12-23 Production Wells No Violations I arrived on site today to conduct a follow up inspection on NOV 1360358200 that was issued on 4/17/2006 for violation of OAC 1501-9-9-05(A) 9.{No SPCC dike/or failure to keep dike free of water or oil}. Previous reports indicated that there was approximately a 12 foot by 40 foot area inside the dike that was contaminated. With my inspection on this date I found that the contaminated area had been remedied. However, with my visual inspection of the production equipment place inside the earthen dike I noticed heavy saturation around the 100bbl. production tank. After performing a field test using a Quantab(Titration testing strips for Chloride), results showed that the area around the tank battery was saturated with water that contained approximately 280 ppm(parts per million) of chloride. The NOV cannot be considered resolved until all pollution is properly accessed. A phone call will be made to the owner/operator informing him of the issue.

1757575315 2018-06-11 Production Wells No Violations On the day of my inspection I found this swab well equipped with 4.5 inch diameter casing and a 2.375 inch diameter changeover. Casing valve is open and the 2 valves on the flowline are closed. Production equipment includes a 100 barrel steel tank that has been painted since my last inspection, horizontal seperator, and a domestic gas meter. The identification was painted over and a call will be placed to operator. Well shows 260 psi. Domestic meter shows 14 psi and is leaking around the gauge. Valves on domestic meter are open, yet valves on well flowline are closed. Operated indicated that they are sending gas from the pipeline to the domestic meter and they will get ID up and leak repaired.
1778520004 2016-07-06 Production Wells No Violations The production tanks remain on site and have not been removed. The operator was to remove them this week. I checked the tanks and the tanks have been drained in preparation to being removed. The well is a swab well and will not be used until the tanks have been replaced.
1817951306 2014-03-19 Production Wells No Violations I arrived today to check on the clean up of the oil and brine spill inside the dike. I found that there has been no effort to clean up the contamination inside the dike. I called Tipka Energy @ 330-339-4411 and left a message on the answering machine with a request to contact me.
2051115654 2016-07-10 Production Wells No Violations Today I returned to check to see if the leaking tanks have been removed and the area under the tanks excavated and the contaminated soil removed. The tanks appear to have been pumped dry and are not leaking at this inspection, the tanks still need to be removed. All of them are to be removed. There are two tanks which were not removed during the last clean up from a leaking tank.
