0010606242 | 2016-11-09 | Final Restoration | No Violations | I am on site today to check on the status of the well after the plug job was completed the previous day. On the day of my inspection I witnessed the earthen dike pit had been vacuumed then filled in, the surface plug had remained at the same level from the previous day and all indications of hydrogen sulfide gas have been remediated. While on site I spoke to the landowner of the well, who stated the rotten egg smell (H2S) had stopped after the plug job was completed and the pit was vacuumed up. After my inspection I spoke to Bill Tipka who informed me that he plans to cut the surface casing and reclaim the location within the next few days. I will be back on site at a later date to perform a final restoration status check. |

0361547088 | 2016-11-23 | Final Restoration | No Violations | I arrived on site today to check the status of the final restoration for the well bore. On the day of my inspection I discovered that the pits have been filled in and the area around the well bore has been graded. The production equipment still remains on site, I will be back to continue checking the final restoration of well bore. Over the duration of my inspection I did not witness any signs of pollution or contamination. |
0538115736 | 2016-11-03 | Plug / Plug Back | No Violations | I am on site today to check on the well bore due to H2S concerns from the plug that was set the previous day. While on site I did not encounter any concerning levels of H2S at the well head or around the pit. However a strong odor was noticed during the duration of my inspection. |
0798298557 | 2016-10-31 | Plug / Plug Back | No Violations | I arrived on location today to meet with Well Owner/Operator Bill Tipka in preparation to plug this well bore. On the day of my inspection I was informed by Bill that he wanted to plug the well due to H2S concerns. Bill stated that one of his well tenders had readings of upwards of 25 ppm of hydrogen sulfide and the well was lacking in production. After being informed about the well Bill and I created a schedule on how the well was going to be plugged. With the conduction of my inspection I discovered the well was utilizing a pump-jack for means of production and had a domestic line tied into the production casing. The well consisted of 8.625 inch diameter surface casing, 4.5 inch diameter production casing and 2.375 inch diameter production tubing. Production equipment consisted of a 210 barrel production tank, a 30 barrel brine tank and a horizontal separator. All production equipment was placed in an earthen dike that measure approximately 30*50 feet with an average height of 2.5 feet around the perimeter. In the duration of my inspection I did not discover any indication of H2S, indicating that the gas was contained inside the 8.625 inch diameter surface casing. Bill also stated that he plans to plug the well within the next 10 days. |
0916198342 | 2016-11-01 | Plug / Plug Back | No Violations | On the day of my inspection I witnessed, Jerry Moore rigging up over the hole with a service rig and pulling all rods out of the well bore. Prior to working on the well bore the well was treated with sulfide blocker and H2S readings were taken on the production casing and surface casing. After removing the rods it was decided that the bottom plug would not be set until the next day. |

-1087 feet using 51 sacks of class a 14.7 pound/1.36 yield cement and displacing with 4.2 barrels. Before the plug was set sulfur clear was mixed and circulation was established. After setting the plug the tubing was then pulled to 220 feet. Formation then circulated cement to surface from a depth of 220 feet utilizing the same cement as before. Prior to setting the plug circulation was established. I witnessed good cement returns to surface and will be back out on site to check on the cement level. | |

1411583782 | 2014-09-24 | Production Wells | No Violations | I received a call from the landowner Robert Henry that he has an idle well which is on his property. I arrived and found that the well is in operation and that it is not idle. I did notice a odor of gas and what could be a low concentrate of H2S. I contacted Brian Tipka and advised that there could be a leak on the casing which would get worse since it is in front of a home at the bottom of a valley. Last I advised that the landowner was hooked up to the well for heat and that he could be in danger if H2S was to be burned in the home heating system. He is to check to make sure H2S is not present at the well and that the home heating system does not use it if there is H2S. |
2079156653 | 2016-11-02 | Plug / Plug Back | No Violations | I am on site today to witness the plugging of the well bore. Jerry Moore is preforming all service work and formation cementing will be preforming all cement work. After removing all rods from the well bore, the tubing was then lifted to assure it was free. After lifting the tubing it was then place back at its original depth. Formation cementing then cleared tubbing at set a plug from 5800-5350 using 35 sacks of Class A 15.6 pound/ 1.18 yield cement and displaced with 20.7 barrels of fresh water. After the plug was set the tubing was then pulled up to a depth of 4150 feet and another plug was set from 4150-3950 using 15 sacks of the same cement and displacing with 15.2 barrels of fresh water. Circulation was established, resulting in an H2S spike will setting the plug. The location was cleared during the displacement of the plug. After the plug was set, the perimeter around the well bore was left until the H2S levels dropped below 10 ppm. Once working conditions were declared safe the tubing was then pulled from the well and shut in for the night. |
