1071234863 | 2018-09-05 | Plug / Plug Back | No Violations | On the day of my inspection I was accompanied by Inspector Bill Stolarik. Nicks Well Plugging is rigged up on location. They were able to thread the tubing. Appalachian Well Service ran in with a 360 cutter to 5990 feet. The charge shorted out so they came out of wellbore. Made necessary repairs and went back in to 5990 feet and shot the tubing. Nicks then pumped 60 sacks of class A neat cement weighing 15.6 ppg. Displacement was 13 barrels. This plug should be from 5900-5200. Cemex is the cement supplier. Appalachian ran back in to shoot the tubing at 3892. They pulled 1 joint of tubing and plan on circulating the well and letting it rest overnight. Next plug will be set tomorrow morning. |

Nicks Well Service rig and Cement Service is on location along with Appalachian Wireline Service. Upon arrival the rig crew threaded the top 1.900 diameter tubing and rigged the pump truck up and attempted to circulate water trough the 1.900 production tubing. The tubing would not circulate so Appalachian Wireline was rigged up and ran in the hole to shoot the mud anchor off the tubing. There 1st attempt to shoot the tubing was unsuccessful due to the explosive shot not going off. A second successful run was made and the tubing was shot at 5,990. The service company then hooked up to the tubing with their pump trucks and made sure the tubing was shot off and fluid would clear out the bottom. They then proceeded to pump a 750 12.6 barrel absolute volume plug consisting of 60 sacks of class A neat cement. The well circulated during the setting of the plug. H2S was present at surface at this point. According to the wireline engineer for Appalachian the tubing was determined to have a tight spot in it at 4,100. I consulted with my Supervisor Mike Moreland and approval was granted to shoot the 1.900 tubing off above the tight spot. The tubing was cut off at 3,892 and one tubing joint pulled to 3,859. The rig crew then decided to stop for the day. |

They have dug below grade and daylighted the wellhead. They attempted to pull 1.5 inch diameter tubing with a spear and it would not grab. They acquired another spear and pulled >20000 lbs. and it did not come free. Appalachian Well Service was onsite for wireline services. The tubing apparently has a small bend in it below the wellhead and they were not able to run a long bar. They ended up running a collar locator with no weight bar to around 6000 feet. Nicks plans on threading the tubing and starting again tomorrow. |
1536243804 | 2018-09-06 | Plug / Plug Back | No Violations | On site inspection to witness day 3 of the well plugging process. Nicks Well Service and Cement Service is the primary contractor. Previously a 750 bottom hole plug was set to isolate the bottom formation and the possible H2S from the Newberg formation. The tubing was still hung after shooting it off at 5,990 and setting the bottom plug so the tubing was shot off once again at 3,892, the tubing pulled free at that point and a 200 plug was set inside the 4.5 diameter casing through the tubing and the tubing pulled out of the hole. Once the tubing was out of the hole a packer was set inside the 4.5 at surface and the 4.5 pressure tested to 750 psi, the casing held pressure so the decision was made to shoot the 4.5 off just above the last plug and plug out from that point. It also was discovered at that point while pulling stretch on the 4.5 that the 8.625 diameter surface pipe well head was moving up hole also. The 4.5 was shot off at 3,487 by Appalachian Wireline service with a 15 shot collar buster shot. The 8.625 diameter casing was rotted off just below the well head at ground level. One 42 casing joint was pulled out of the hole so the crew could get on a good collar and a 12 barrel absolute volume class A plug set, the plug consisted of 58 sacks of neat cement and was displaced with 51 barrels of pit fluid. Due to high H2S the decision was made to shut down and not attempt to pull the casing out of the plug with the understanding they would rip the casing back off in the morning after the H2S has subsided. |

Prior to my arrival the service rig company identified as Nicks Plugging LLC ripped the 4.5 diameter production casing off at 1,630 and pulled one 4.5 diameter casing joint. The top of the last plug set was calculated at 200 above the point the casing was shot off on the previous working day. The 4.5 was shot off at 3,487 and a 200 isolation plug was set, the rig was unable to pull the 4.5 due to extremely high levels of H2S gas migrating to surface. The company man on site was directed to tag the plug and re rip the 4.5 above the plug top at 3,287. The service company set a 250 cement plug to isolate the bottom of the 8.625 surface casing. The 4.5 was recovered from that point. I informed bot District Supervisor Brent Bear and Regional Supervisor Mike Moreland that the service company did not rip the 4.5above the last plug as indicated on the previous day. |

On this day the previous cement plug set across the bottom of the 8.625 diameter surface pipe was tagged at 1,390. At that point the 8.625 was topped of with fresh water to surface and determined to be static. A 105 sack class A surface plug was then set through 1.900 tubing from 210 to surface. The 1.900 was pulled up and did pull wet with cement. The well stayed static with cement to surface. The landowner was on location and informed the production tank would need removed and vegetation established at bot the well and tank battery site once restoration grading is complete. |
