0207814453 | 2016-04-06 | Plug / Plug Back | No Violations | On site today accompanied by Inspector Mark Moreland to witness the plugging of this well bore. Cox Well Service will be providing a crane truck to run the 1.5 inch diameter tubing to set the plugs. Cemenco is the cement contractor preforming the cement work. Cox Well Service ran tubing to a depth of 590 feet to tag the bridge plug set inside the 4.5 inch diameter production casing (bon-log indicate cement was present on the backside all the way to surface) Appalachian Well Service set prior to the day of my inspection. Cemenco then established circulation and displaced cement to surface. Mark Clark (The contractor hired by the well owner to plug these wells) stated he would be back at a later date to top off the well bore with cement and cut the pipe 3 feet below ground level. |
0600619112 | 2016-03-15 | Plug / Plug Back | No Violations | I am on site accompanied by Inspector Mark Moreland to check on the static test of the well bore. A bridge plug was place inside the 7 inch diameter casing the previous day, in preparation for plugging the well bore. After the bridge plug was set the hole was then filled with fresh water to test the integrity of the bridge plug. On the day of my inspection I found that the bridge plug held indicating that the well could then be filled to surface with cement. |

1315839782 | 2016-04-04 | Plug / Plug Back | No Violations | I am on site today accompanied by inspector Mark Moreland, to witness Appalachian Well Service running a bond-log. The bond-log was ran a request by inspector Moreland, when the owner of the well stated that 4.5 inch diameter production casing was ran and cemented in place. The company provided documentation that indicated the 4.5 inch diameter pipe was placed in the well bore and cemented in place after an open hole collapse had occurred. However the documentation was not found in RBDMS records. With the conduction of the bond-log Mark and I witnessed the pipe was properly cemented. Mark then gave the approval to plug the well by using the bridge plug that was set in that 4.5 production casing and filling to surface. |
1434970579 | 2016-07-13 | Final Restoration | No Violations | On the day of my inspection, I found that the final restoration has been complete. The site passed final restoration according to ORC 1509.072(B). |
