1444909188 2016-04-06 Plug / Plug Back No Violations On site today accompanied by Inspector Mark Moreland to witness the plugging of this well bore. Cox Well Service will be providing a crane truck to run the 1.5 inch diameter tubing to set the plugs. Cemenco is the cement contractor preforming the cement work. Cox Well Service ran tubing to a depth of 510 feet to tag the bridge plug inside the 7 inch diameter surface casing Appalachian Well Service set prior to the day of my inspection. Cemenco then established circulation and displaced cement to surface. Mark Clark (The contractor hired by the well owner to plug these wells) stated he would be back at a later date to top off the well bore with cement and cut the pipe 3 feet below ground level.
1459875801 2016-04-04 Plug / Plug Back No Violations Appalachian Well Service is on site to set a wire line bridge plug inside the 7 inch diameter surface casing in preparation for plugging this well. Appalachian hit an obstruction in the hole at 520 feet. The last collar for the 7 inch diameter pipe was found at 515 feet. The bridge plug was then set at 513 feet. The well will be filled with cement on a later date. I was accompanied by Mark Moreland on the day of my inspection.

1692848185 2016-07-13 Final Restoration No Violations On the day of my inspection, I found that the final restoration has been complete. The site passed final restoration according to ORC 1509.072(B).
1933228568 2008-01-29 Production Wells No Violations Inspection revealed no violations at this time. Note: Coal Bed Methane well.
2049801346 2016-04-12 Plug / Plug Back No Violations On the day of my inspection, I witnessed that the final restoration could not be passed. The lease road had not been restored, the well location needed to be graded and vegetation had not been established. A call will be placed to discover when the location will be restored.
