0459605837 2014-01-29 Preliminary Restoration No Violations On the Day of my inspection the site passed preliminary restoration according to 1509.072A.
1169469302 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells 34067210640000 and 34067210630000 are utilizing a plunger lift system. RBDMS production records indicate that the wells are in production. Permitted wells 34067210770000 and 34067210840000 have not been drilled, but the tin horns have been set.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 2 Producing wells 2 Sand separators

2 Horizontal heater-treaters 1 combustor 1 compressor 1 Low pressure separator tower/Vapor recovery system unit. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1339571116 2012-06-12 Drill / Deepen / Reopen No Violations Division of Oil & Gas Rersources Management Inspector Shawn Murphy and I were on site to check the drilling status of Patterson Rig # 259. Our inspection revealed that they have reached a total depth of 2675 on their 12 1/4 hole. They are rigging up the loggers to run the cement bond log on the 13 3/8 casing. They will also run a caliper log and a triple combo log. The Drilling Consultant , Doug Northcott, advised that it would take at least seven hours to run all the logs and then they would be ready to start rigging up the casing crew to run the 9 5/8 casing. I advised Doug Northcott that I would call him later this evening to check on the progress of the logging and the rigging up of the casing crew. Our inspection revealed no violations at this time.
1507812756 2017-10-12 Production Wells No Violations On the day of my inspection I found the pad to contain two plunger lift wells and a conductor pipe inside of a cellar. Production equipment includes six 400 barrel production tanks, two vertical seperators, two GPUs, one combustor and a vapor recovery system.
