0257558484 2014-06-20 Production Wells No Violations On the day of my inspection I found that the release had been in the northeast corner of the containment dike. I found no condensate on the production equipment or the liner of the containment dike. I found a waste disposal container from Kimble on location that was full of soil and gravel that had been excavated from around the dike. I found new gravel down along the north and east walls of the dike. I inspected the soil under the new gravel and I found no viscosity or odor that would be indicative of condensate. I took photos of the clean up and I contacted Kris Andersen to notify him that the clean up was satisfactory.

0950561424 2013-09-30 Preliminary Restoration No Violations On the day of my inspection the well was in production and the site passed preliminary restoration according to 1509.072A.
1258458437 2016-03-07 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells on site are producing off of natural reservoir pressure. Permitted wells 34067211040000 and 34067211050000 have not been drilled. RBDMS indicated that the wells on site are in producing order.
The production equipment on the pad consists of the following:

14 400 bbl. Production tanks 4 Producing wells 4 Sand separators

4 Line heaters 1 compressor 1 Vapor recovery tower 1 combustor 1 30bbl. drip tank The site passed preliminary restoration in accordance with ORC 1509.072 A.

1342205289 2012-07-11 Completion Testing No Violations DNR Inspector Mark Moreland and I were on site to check the status of the well. The well has been fracked and the operator has it shut in until later this year, after there is a decrease in the activity at the Boy Scout Camp. As noted in the previous inspection report, the entire well site has been enclosed with chain link fence. Inspector Moreland and I noted no violations during our inspection.
1433540165 2015-06-05 Production Wells No Violations I received a call at 1:40 PM from Brad Burwell and he advised me that a condensate leak was detected on the Boy Scout Well Pad. He said that the leak was on the 2 inch dump line running between the vertical Separator and the production tanks. The line runs along the far side of the pad toward the road next to the fence. The line was reported as a poly line. When I arrived and the line was excavated it was determined that it was a steel line. The line appears to be leaking at the threads on each end of the 20 ft. line. The leak was detected while they were digging up the buried lines and moving everything above ground. At this time Hall and Associates is on site using a Photo Ionization Detector to screen the soil as the area is excavated. The area is next to the fence and the lay down separators. The area is 18 ft. long 2 ft. wide and 1 -1/2 ft. deep at this time. Work is to continue thru the next few days until the area is clean. Samples are to be sent to the lab for confirmation.
1433624147 2015-06-06 Production Wells No Violations I arrived today to find that digging continues near the vertical heater treater toward the fence line. There has been no definitive source of the condensate leak. The area near the heater treater then 12 to 15 ft. in a left right direction along the fence line 10 ft. wide and 4 ft. deep has been excavated. The total amount of soil removed should fill 3 to 4 roll off containers. Due to the excavation around the vertical heater treater it will need to come down and the area under it excavated. Tests on the underground lines are to be done on Monday and may point to a source for the leak. Hall and Associates are on site again today and have dug bore holes to test for condensate with a Photo Ionization Detector and show two sides as clean. The clean up is to continue until all of the contaminated soil has been removed. Work could continue for several more days.
1433770936 2015-06-08 Production Wells No Violations Clean up continues today with the removal of the Vertical Heater Treater and several line heaters. A Crane is scheduled to arrive to remove the equipment so that the area under them can be excavated. Rain is also a concern because heavy rains are in the forecast for today and tonight. Arrangements have been made to cover the area that has been excavated. All of the contaminated soil has been placed in roll off containers and there should be no off site impact due to the weather.
1439493965 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains four properly labeled producing wellheads. Production equipment includes five 400 barrel steel tanks in their own polymer lined steel containment, four GPUs, four sand seperators, four vertical seperators, and a vapor recovery unit. Surface annular pressure is 0 and intermediate annular pressure is 0. Production records are current in RBDMS.
1718968085 2015-06-09 Production Wells No Violations I arrived on site today to find that the vertical heater treater has been taken down along with several line heaters. I spoke to Brad Burwell and was informed that all of the production equipment in place was to be moved over and the lines buried below ground removed. That area will also be checked with the PID meter to make sure it is free of contamination. There are 5 to 7 roll off containers that are full at this time.
1771444553 2014-06-19 Production Wells Violations Noted I received phone notification from Kris Andersen, of AOL Consulting, that there was a condensate release at the tank battery. Mr. Andersen reported that <0.5 barrels were released from a faulty valve inside the dike. He estimated that 5 to 7 gallons ran down the dike liner, contaminating the gravel outside the dike. He claimed that no condensate got off of the actual well pad. They had an excavating crew on location to remove any impacted soil and gravel, as well as a steam wash and vacuum truck to decontaminate the inside of the containment dike.
