0329166783 2015-07-30 Production Wells No Violations There was a corner of the pad which had some under cut erosion occurring on the township road side of the location. The erosion has been stopped and the area now re-graded and seeded. I wanted to check this site for any updates that Gulfport is making to the production tank containment dike.

0585487231 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming. This pad contains four properly labeled producing wellheads. Production equipment includes twelve 400 barrel steel tanks in their own polymer lined steel containment, one 50 barrel steel tank, four GPUs, four sand seperators, and a combustor. Surface annular pressure is 0 and intermediate annular pressure is 0. Production records are current in RBDMS.
0746824517 2013-12-10 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration according to 1509.072A.
0990797198 2012-06-27 Drill / Deepen / Reopen No Violations I was on site because Dave Lewton (NewTech) contacted me and advised that while drilling the Clinton formation they lost circulation and lost approximately 400 barrels of oil base mud in the Clinton. He also advised that they would be running a 7 intermediate casing string thru the Clinton formation and then they would slim hole the well to total depth. My inspection revealed that UDI Rig #60 has TDed the hole at 7010 and ran 6985 of 7 casing. A two stage tool (DV Tool) was installed in the casing string at 5794. Halliburton cemented the bottom of the 7 casing, shut in, opened the tool and started circulating thru the ports in the tool. Charlie Morgan with NewTech advised that they were going to circulate for at least four hours after Halliburton is finished circulating. He also advised that they would do the second stage of the cement job and cement the casing to surface. I departed the well site and went back to the plug job on permit number 0123 in Harrison County.
1216864986 2012-07-11 Drill / Deepen / Reopen No Violations DNR Inspector Mark Moreland and I were on site to check the status of UDI Rig # 60. Our inspection revealed that the rig is drilling at 13369 and drilling ahead in the lateral section of the well. We spoke with Dave Lewton (Consultant) who stated that they shoul reach total depth in a couple of days. Our inspection revealed no violations at this time. We departed the site and went to permit number 1072 (Gulfport Energy- BK Stephens).

1339987985 2012-06-11 Drill / Deepen / Reopen No Violations Division of Oil and Gas Resources Management Inspector Shawn Murphy and I conducted an inspection on this site to check the status of Union Drilling (UDI) Rig # 60. Our inspection revealed that they are drilling on air at 4475. We advised Charlie Morgan ( Drilling Consultant) that there is a possibility of drilling into a hydrogen sulfide gas bearing formation between now and when they drill into the Bass Island formation. He advised us that he has read the permit conditions and was aware of a possible H2S gas situation occurring. Inspector Murphy and I observed plastic ribbons tied in different areas of the location to indicate air flow in case H2S gas was encountered while drilling. Our inspection revealed no violations at this time.
1414843431 2014-10-31 Production Wells No Violations I received a call around 7 PM from Brad Burwell to inform me that a transport vehicle developed a leak on the sight tube. It allowed condensate to spill onto the limestone surface out of containment. The call came in from the lead pumper on site James Noyse. The spill is less than 1 bbl. And has spread out to a 10x10 ft. area. When I arrived they had a Super Sucker vacuum truck on site and it had picked up the condensate and contaminated limestone. I advised that the material be disposed of at a proper landfill. Update Supervisor Bear with Spill report and e-mail.
1419005155 2014-12-19 Production Wells No Violations I received a call from Brad Burwell on 12/18/2014 at 7 PM to inform me that there had been a spill on the Ryser pad. It was from a buried poly line that was discovered leaking after blowing condensate to surface. The line had been pressure tested prior to this leak. They think the leak could not be more than one day. I contacted Supervisor Bear to update him on the spill and advised that I would investigate the spill on Friday. I arrived Friday and found that they have Hall and Associates on site to help determine the contamination area. They are using a PID meter to screen the excavation of material in the production line area. The spill area is toward the back of the production facility near the side next to the road. The leak occurred in the middle of the area where the stand up separators and line heaters are located. They are placing the contaminated soil on a plastic liner until roll off containers can be dropped off. The dig site today is 20 ft. to 25 ft. long 4 to 5 ft. wide and 3 to 4 ft. deep. Digging will continue thru the weekend or until the contamination has be excavated and removed.
1420033042 2014-12-30 Production Wells No Violations The condensate spill has been cleaned up and the area excavated has been filled, graded and rocked. The contaminated soil has been removed and the wells are back in operation.
1795881775 2012-07-16 Drill / Deepen / Reopen No Violations I arrived today to check on the drilling depth and well status. I found that U.D.I. Rig # 60 has reached total depth and has completed drilling. The total depth is 15,967ft. The well should be ready to cement this evening around 4 pm. I did return at 4:45 pm and witnessed the running and cementing of the 5 inch casing. Halliburton Cemented the longstring casing with 280 sks of Varcem cement. This is a latex cement. They also cemented with 540 sks of Halcem cement for a total of 820 sks. The top of the cement went up into the liner set thru the Clinton to a height of 2000 ft.
