1024740314 2012-12-21 Drill / Deepen / Reopen No Violations I had a follow up call from Deputy Chief Bob Worstall. Bob wanted to know if there was another problem on this site or one leaving this site. I gave Bob the findings that I had up to the time of the call. I advised I had been on site again and that I had met with Chesapeake, Nomac Drilling and OEPA on site 12/19/2012. I advised Deputy Chief Worstall of my meeting on site with Aaron Wolfe of OEPA from Storm Water Southeast District. Bob wanted to know if they had any problems with the water quality. I told Bob that Aaron does not plan to pursue any issues in regard to this site. I also advised that there is the same material on the road surface as the well site. There is crushed limestone on Dodson road and County Rd 17 which has just been black topped a few days after the 1st call about a problem with off site water quality.
1321573938 2013-09-26 Preliminary Restoration No Violations On the Day of my inspection the site passed preliminary restoration according to 1509.072A.
1352397538 2012-12-10 Drill / Deepen / Reopen No Violations I am on site today to witness the running and the cementing of the 13-3/8 diameter casing string. Nomac Drilling rig # 311 set 508 ft of casing and O - Tex Cementing Service circulated cement to surface. I reviewed the water diversions along with the run off controls for this site. Most of the water control is along the entrance road to the drill site. I discovered 4 to 5 inches of rain water on the drilling site containment pad. The containment pad is 150 ft x 150 ft. x 12 inches. An hour or so after leaving this site I received a phone call from Inspector Farnsworth about a call he had about this site. He said the call was strange but maybe I should call to clarify. 740-945-0217 I did make a call to that number. I was not able to ask all of the questions needed to find out all of the details before the person hung up. I did a reverse call look up and found that it was from John Luber of 86663 Fife Rd. Cadiz , Ohio 43907
1356364600 2012-12-19 Drill / Deepen / Reopen No Violations I am meeting today with 4 members of Chesapeake Exploration along with Nomac Drilling rig # 311 company man. I went over all of the information I had about a possible problem of contamination leaving the site. I talked with both parties about the water control measures and advised that I could not find a problem with contamination or sedimentation leaving the site. Last I met with Aaron Wolfe of OEPA Storm Water Coordinator and reviewed his and my findings. He advised me he would not continue any further action because he had not found a problem.
1359510273 2013-01-29 Production Wells No Violations I am on site today to see if any further erosion is occurring along the lease road from the entrance to the well pad. The water looks less cloudy today than on prior inspections. I am checking this because I received a call from the Harrison Co. Health Dept Charles Fisher. He advised me that the seals on the wells are leaking material into the stream below. He said that it was not run off from the site but was the material used to seal the wells that was leaking. I also advised Deputy Chief Claus on the phone conversation, he is to return a call to Charles Fisher.
1455800215 2016-02-17 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. Permitted well numbers 34067211160000, 34067211100000 and 34067211090000 are utilizing a plunger lift system for means of production. While permitted wells 34067212910000, 34067212920000 and 34067210920000 are producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 6 Producing wells 6 Sand separators

1 Low pressure separator tower/Vapor recovery system unit. 1 Combustor 1 Gas compressor unit. 2 Horizontal heater-treater. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1791097049 2012-12-20 Drill / Deepen / Reopen No Violations I received a call from District Supervisor Rocky King. He asked what had been done in finding the problem at this site. I gave him the facts of my inspections and went over the history of the call. I advised him of the call with John Luber and that he had not made a complaint but had hung up on me before I was able to complete my questions. He advised me to complete a complaint log update with the information I had. I was advised to include all information I shared with OEPA, Health Dept., Chesapeake Exploration and Nomac Drilling rig # 311.
