0130396781 | 2013-10-22 | Production Wells | No Violations | I am on site today to check on the operation status of the well. I found that it was in production and did not find any violations on this site at this time. |
0693251413 | 2018-03-19 | Production Wells | No Violations | On the day of my inpection I found the Lyon pad to have 3 properly labeled and producing plunger lift wells. This well has 200 psi on the surface casing and no pressure on the intermediate casing. Production equipment includes four 400 barrel steel tanks and one 50 barrel steel tank, all are contained in a steel dike. Three sand seperators, a heater treater, and a combustor are also onsite. |

1397523054 | 2014-04-14 | Production Wells | No Violations | I am on site today with David Lee of Gulfport Energy along with Randy Akins from ODNR. We are responding to a complaint from Stephanie Rohal. She said that this production facility is generating tremors and that they increase during the night as more equipment is operated. My inspection did not reveal any tremors or loud sounds. I was able to talk on my cell phone from any point near the equipment. I called Stephanie to clarify her complaint and she did not return my call. I contacted Bob Worstall and ask for seismic records for that time period during the night. I was advised that records for seismic events did not reveal any activity for that time period. |
1402340659 | 2014-06-09 | Production Wells | Violations Noted | I received a call today from Chris Anderson to inform me that a spill had occurred on the Lyons Pad. He said that there had been a line which broke on the compressor station and blew condensate and light crude over an area 150ft x 150ft. When I arrived on site I did find that crude and condensate had been spilled on the site. I reviewed the site with Chris and ask what type of equipment was called to cleanup the spill. Then a berm was established to act as a barrier to any fluid exiting the site. There was a ditch cut and filled with peatsorb and in front was to be an absorbent boom along the entire length. That same thing was repeated near the back side of the production tanks and compressors to make a two sided containment. The area is to be vacuumed free of fluid and then pressure washed into a sump at the low point of the pad in front of the line heaters. Containment is also being set up to hold the rock and soil removed before a roll off container can be dropped off. All of the excavated rock and soil is to be removed to a landfill. |
1402492898 | 2014-06-10 | Production Wells | No Violations | I arrived on site today to find the cleanup of the contaminated soil and rock from a spill 06/09/2014 continuing. 50 % of the contaminated soil and rock has been removed. I collected photos to show the progress. All of the free fluid has been removed and containment is in place to capture any fluid before it can exit the production pad. Chris Anderson is on site and advised me that they worked through the night to make sure containment is in place and to remove as much contamination as possible before a rain event. He has vacuum trucks on site to deal with any rain and has duck pond containment set up to store excavated material until it can be loaded into a dump truck and removed to a landfill. Clean up is to continue through the night again until all of the contamination has been removed. The wells on the site are not in operation at this time and will remain shut in until the cleanup is complete. |
1402606085 | 2014-06-12 | Production Wells | No Violations | I arrived today to check on the status of the cleanup of the spill from 6/09/2014. The entire area that had contamination from the spill has been cleaned up and hauled to Kimbles Landfill .Clean material is being compacted and put back in place of the excavated material. There has been no run off of contamination from the site. During the clean up there has been no rain events to cause material to exit the site. Containment is to be left in place until after a few rain events have occurred just to make sure nothing has been missed during the cleanup. I checked the two sumps near the entrance for chlorides with a titration test strip and had no readings for the second day in a row. |
1403016881 | 2014-06-16 | Production Wells | No Violations | I am on site today to check on the cleanup status of the spill which occurred 06/09/2014. I found that all of the contaminated soil and rock has been removed replaced and graded. The well is back in operation and the equipment used to produce the wells on this pad are operating. I have had calls about noise and vibrations coming from this equipment but can not find any problem at this time. I was able to make a phone call 5 feet from the compressor. I did not have any trouble hearing the conversation during that call. I did check for loud noise and vibrations but did not find the levels to be in excess of normal operations. When I was 100 ft from the compressor I could not be sure that it was running. |
1623425349 | 2014-06-11 | Production Wells | No Violations | Check on cleanup from spill 6/09/2014 . I found that the entire area of the spill has been excavated and removed to a landfill. The area now has clean limestone rock put back in place and the area has been graded. There are two roll off containers on site which need removed and the equipment needs put back in place. The well are not in operation at this time but should be sometime today. I have collected photos today to show the progress of the cleanup. |
1902217994 | 2013-09-25 | Preliminary Restoration | No Violations | On the day of my inspection the site passed preliminary restoration according to 1509.072A. |
