0323850974 2016-02-26 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 2 Producing wells 2 Sand separators

2 Horizontal heater-treaters 1 Low pressure separator/ Vapor recovery system 1 Combustor The site passed preliminary restoration in accordance with ORC 1509.072 A.

0382632118 2018-08-13 Production Wells No Violations On the day of my inspection I witnessed this pad to contain two properly labeled producing wells. Surface and interediate annualr pressures are 0. Production records are current in RBDMS. Production equipment includes three 400 barrel steel produced water tanks, five 400 barrel steel condensate tanks, two sand separators, two GPUs, a compressor, and a combustor. Tanks are inside their own steel containment.
1259148032 2013-04-25 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Precision Drilling has drilled a 26 inch diameter hole to 120 ft and set 120 ft of casing. O- Tex Cementing Service c/w 220 sks and circulated cement to surface
