0045937927 2015-05-28 Preliminary Restoration No Violations On this date I am on site to conduct an inspection on the well site for Preliminary Restoration. The well passes for Preliminary Restoration per ORC 1509.072(A).
0062587079 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

0990814784 2014-09-30 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by company man Joe Hebert that the Patterson 261 rig was drilling an 8.75 inch diameter hole and were at a depth of 5888 feet and tripping out of hole.

1788230431 2014-09-26 Casing String Witnessed No Violations On the date of inspection I was informed by Chesapeake company man Michael Smith that the Patterson 261 rig had drilled a 17.5 inch diameter hole to a depth of 595 feet. Wyoming Casing Crew set 13.375 inch 54.50lbs/ft casing to a depth of 585 feet. The rig established circulation prior to the cement company arriving on site. O-Tex Pumping LLC pumped 540sk of Class H cement, 15.6ppg, 1.2 yield. Additives were 2% CaCl, 0.4% C-41P, and 0.25lbs/sk of Celloflake. 25bbl of cement returned to surface.
