0645794594 2014-07-02 Casing String Witnessed No Violations On the date of inspection, I accompanied ODNR Inspector Mark Moreland to witness the running and cementing of the 20 conductor casing. Express Drilling drilled a 26 hole to a depth of 120 and set 120 of 94# casing. Express Cementing Service cemented with 230sks Class A at 15.6# and yield of 1.18. Company man present on site is Brett Farnsworth. Express Energy Services established circulation in hole before cementing and received 8bbl of cement back to surface. Cement stayed to surface while I was on site.

0961670394 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 low pressure separator/ Vapor recovery system 4 compressors 2 NGL tanks The site passed preliminary restoration in accordance with ORC 1509.072 A.

0968943545 2014-09-16 Drill / Deepen / Reopen No Violations On the date of inspection I was informed by Company Man Rodney Bean that the Patterson 261 rig had drilled an 8.75 inch hole to a depth of 7437 feet. The rig was currently preparing its Bottom Hole Assembly for its curve and lateral. I informed Rodney Bean that I had located a tear in the secondary containment around the rig chemicals. There was approximately a 12 inch tear through the liner along the berm. Rodney will be dispatching a crew to repair the liner.
1308120801 2014-09-10 Casing String Witnessed No Violations On the date of inspection I was informed by Chesapeake company man Rodney Bean that the Patterson 261 rig had drilled a 17.5 inch diameter hole to a total depth of 594 feet. Costvs Energy Servies then ran 13.375 inch casing weighing 54.5lbs/ft to a depth of 586 feet. O-Tex Pumping, LLC (O-Tex) pumped 540sks of Premium H Cement weighing 15.6ppg with a yield of 1.2. Additives include 2% CaCl, 0.4% C-41P, 0.25lbs/sk of Celloflake. O-Tex pumped 84.7bbl of fresh water to displace cement. 28bbl of cement returned to surface.
