The Hess company representative on site was Renee Cowell, EHS & SR Supervisor. The contractor on site was Envirotrac with representatives Dennis Furmanek and Diane Webb. Envirotrac and Hess attempted to use an old liner they had stored to cover the contaminated area. The liner was found to be in poor condition so a new one will be delivered the following morning. Envirotrac performed field screenings of soils in the ditch area with a PID and readings ranging from 0.0ppm to 6390ppm. They were unsuccessful in attempts to procure a geoprobe to delineate the contamination range as well as an excavator to dig a sump in the ditchline to prevent down gradient contamination. They plan to have both pieces of equipment on site as well as utilities marked first thing in the morning. Envirotrac and Hess did not show up on site until 14:00. |

Hess representative on site was Brian Lockwood. Envirotrac Project Specialist on site is Dennis Furmanek. At the time of my inspection I witnessed that a heavy duty liner had been placed across most of the known contaminated area including between heated separators. The up gradient culvert and the down gradient culvert have been sealed with silicone and plywood to prevent water from leaving site in the event of a storm. A pit approximately 3 feet by 3 feet was excavated within the drainage ditch in front of where the spill took place. At the time of my inspection Envirotrac was in the process of delineating the contaminated area using a geoprobe. The geoprobe contractor is Geo Environmental Drilling Company from Pittsburgh, PA. |

1620961742 | 2014-08-14 | Production Wells | No Violations | I received a call from Hess Ohio Development Rene Cowell that a spill of Defoaming agent had leaked at the production facility. The Defoaming agent is very hazardous and requires a Haz Mat crew to respond. The crew responding to the spill is Weavertown and they are to mobilize and be on site today around noon. Myself and Inspector Brandon Trigg went to the site and met with the Weavertown Crew along with Rene Cowell and Brian Lockwood both from Hess Ohio. They reported that a spill of 20 to 150 gallons could have occurred. The spill went undetected because the line carrying the Defoaming agent was below ground level and out of site. It could have leaked a day or more before being detected. When Inspector Trigg and I arrived we found that the area in front of the line heaters contained most of the spill in the dirt under and next to the line heaters and in the road ditch in front of the Defoamer tanks. We witnessed Weavertown remove soil under the Defoamer tanks once they had been removed and in the road ditch. Booms and absorbent pads were placed in the ditch and in front of the line heaters to absorb some of the free defoamer while still liquid. After a very short time, digging, on site was stopped once it became clear that it was too close to high pressure lines and electrical lines. More info is to be gathered and a plan on how to continue developed. Samples are to be collected and shared with ODNR by Weavertown. |

They are to review the spill and submit a plan for remediation to ODNR as soon as possible. They are to collect samples to determine the extent of the area contaminated and share the results with ODNR. |
