0099134556 | 2013-10-09 | Drill / Deepen / Reopen | No Violations | I am on site today to check on the drilling status of H & P Drilling Rig # 284 and found that they are at 6700 ft drilling down to 7850 ft to set 9-5/8 inch diameter casing. |
0305882234 | 2015-01-27 | Production Wells | No Violations | I am on site today to collect photos of the well set up and check to see if this well is being monitored while the Cadiz D # 3 well is being fracked. |
0799902694 | 2015-02-02 | Production Wells | No Violations | I am on site today for two reasons. There was a spill 1/31/2015 of about 40 bbl. Of produced brine. The spill occurred when a truck driver hauling the brine did not close a valve and it leaked outside of the containment. The area covered is about 40 yards x 40 yards as reported to me by Benjamin Badon. Envirotrac is responding to the spill and will collect samples to show that the spill has been cleaned up. Today when I arrived I did not see any ongoing clean up. There is a section of the well pad that has yellow tape around it but does not have anything on the surface like absorbent pads or peat sorb. I am also on site today to check on the status and the continued monitoring of this well while the Cadiz D wells are being fracked. |
1371078406 | 2017-08-31 | Preliminary Restoration | No Violations | On the day of my inspection I found the Cadiz B pad seperated between a production equipment pad and a wellhead pad. Five properly labeled chritsmas tree style welheads are on location. Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, five line heaters, one seperator, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A. |
1376084051 | 2013-08-09 | Drill / Deepen / Reopen | No Violations | I arrived today to check on the drilling status of H & P Drilling Rig # 284 and found that they are at 435 ft on the surface hole but are having trouble getting out of the hole and have lost circulation. They will contact me when they get casing started in the hole or near bottom. |
1376478152 | 2013-08-14 | Casing String Witnessed | No Violations | I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing. H&P Drilling Rig # 284 drilled a 17-1/2 inch diameter hole to 1955 ft and set 1959 ft of casing. Halliburton is on site to cement the well. Had circulation problems while mixing the cement and was able to re-establish and continue. |
1382380299 | 2013-10-21 | Drill / Deepen / Reopen | No Violations | I am on site today to check on the drilling status of H & P Drilling Rig # 284 and found that they are drilling at 8043 ft on fluid. I checked the location and found that the area around the drilling operation looks free of spills or contamination. I checked on the containment pad and found that it is place with most everything that can leak with a secondary containment system in place. |
1383322114 | 2013-11-01 | Drill / Deepen / Reopen | No Violations | H & P Drilling Rig # 284 is at 15,845 ft drilling Point Pleasant . Should TD Tues. and move to the 4H next. |
1414789393 | 2014-10-31 | Production Wells | No Violations | I received a call from Marcus Attaway of Hess Ohio and he told me that he was the Completion Supervisor for Hess Ohio. He was calling to inform me that there was a fire involving a Halliburton Pump truck. The pump truck was on site conducting stimulation of the well. During that operation it caught fire from a hydraulic line leak. The fire was brought under control with several fire extinguishers on site with the trucks. Cadiz Fire Department was called as a precaution in case the vehicle reignited. The Cadiz Fire Department remained on site and left after they were sure there was no further danger. There are 5 wells on this drilling pad, all are shut in. There were no injuries and nobody required medical attention. I contacted Supervisor Brent Bear to advise of the fire. The incident occurred around 12:15 PM this Friday. |
2114009267 | 2013-11-18 | Drill / Deepen / Reopen | No Violations | I am on site today because I had a call to inform me that they would be running casing and cementing today. I found that H & P Drilling Rig # 284 drilled to 18,248 ft but could not get casing to that depth. After 3 days of washing and conditioning they are now at 14,363 ft and will try to cement the well at this depth. |
