0966908442 | 2013-11-21 | Drill / Deepen / Reopen | No Violations | HAD Drilling Rig # 690 is on site today drilling the 30 inch diameter conductor section of this well. The casing does not have to be cemented because there is no annulus. |
1081881238 | 2013-12-16 | Casing String Witnessed | No Violations | I am on site today to witness the running and the cementing of the 18-5/8 casing string. Today I could only watch them install three joints and then they dropped it down the well. The cement job was then cancelled for the day. Casing fell to 520 ft and now will need to be fished out of the well or the well will need plugged if it can not be retrieved. |
1130522595 | 2015-05-19 | Preliminary Restoration | No Violations | On the date of inspection I am on site to conduct an inspection for preliminary restoration. The well passes for preliminary restoration per ORC 1509.072(A). |
1164581829 | 2016-06-14 | Preliminary Restoration | No Violations | I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following: 4 500 bbl. Production tanks 4 Producing wells 4 Sand separators 1 Combustor 1 Compressor The site passed preliminary restoration in accordance with ORC 1509.072 A. |
1201735416 | 2013-11-19 | Drill / Deepen / Reopen | No Violations | HAD Drilling Rig # 690 is on site today and will be rigging up to drill 4 wells on this pad. Met with the Company Man for Hess Ohio and they plan on setting conductor and the Mine String with this rig and move another rig in to set Surface Casing. |
1358038038 | 2013-11-26 | Drill / Deepen / Reopen | No Violations | I am on site today to check on the drilling status of the HAD Drilling Rig # 680 and found that they are at 120 ft on the conductor hole and should be done today with the conductor. Next they will start on the Mine String hole which is 200 to 220 ft deep. |

1535234625 | 2015-01-20 | Production Wells | No Violations | I am on site today because I received a call to inform me that a spill had occurred on Monday which was a Holiday. The spill was from a valve over flow which has been left open by a truck driver. The valve spilled approximately 55 gallons of produced water, or brine unto the road surface and spread across the road where it puddled into a low spot. The soil and fluid has been removed and there is no sign that a spill occurred. While conducting my inspection I did find a number of 55 gallon drums inside two small duck pond type containments. I collected photos and forwarded them to Hess Ohio at 281-536-9170 Ben Badon who is helping Dave Boden on the clean up. I advised that the containment for these drums was not adequate because they could fall over and then would be out of containment. Also too many drums for the tiny container. He advised me that the drums are to be removed and that they would update this practice so it did not occur on other sites. |
1603466113 | 2014-02-11 | Casing String Witnessed | No Violations | I was advised that the well would begin cementing casing this morning. When I arrived I discovered that there was a problem with the Cementers and the arrival time. The job now could be several hours from beginning. |
1682645474 | 2017-12-11 | Production Wells | No Violations | On the day of my inspection I found this pad containing 4 producing wellheads. Wells heads are on a separate pad from production equipment. Production equipment includes four 400 barrel production tanks, four GPUs,one compressor, and flare. Well is showing 130 psi on the surface casing and 130 psi on intermediate casing. Valves are open and production records are current. |

1941700218 | 2016-09-07 | Production Wells | No Violations | On the day of my inspection I witnessed the lease road gate was locked and the lease road in adequate condition. The permitted well is producing off of natural reservoir pressure. RBDMS records indicate that the well is in producing order. The production tubing is being monitored using a digital Psi gauge placed at the top of the well head. At the time of my inspection the production tubing gauge read approximately 890 psi. Fluids and gases could be heard transferring from the wellhead to the flow line. Indicating the well is producing. I was unable to capture the pressure readings of the surface casing and intermediate casing due to the fact it was not clear what position the valves were in. The gases coming from the tanks were being flared off.
Production equipment on the well pad consisted of the following. 4- 400 barrel production tanks 1- Condensate compressor station. 4- Gas Production Units with built in line heaters and sand separators 1- Flare stack. 4- Producing wells utilizing natural reservoir pressure. |
1956389323 | 2013-12-12 | Drill / Deepen / Reopen | No Violations | HAD Drilling is at 250 ft on the surface hole and will run casing and cement later tonight. |
1966984501 | 2013-11-22 | Drill / Deepen / Reopen | No Violations | HAD Drilling Rig # 690 is a new drilling rig and because this is the first hole they are having some equipment issues. |
2138971263 | 2014-05-07 | Drill / Deepen / Reopen | No Violations | On the day of my inspection I received a call from Harrison County ODNR Inspector Mark Moreland whom received a call from Hess about Chlorides reading 1500ppm on the north side of the pad. Upon arrival I did a walk around with Company Man Bill Calabreeze and observed the area which has been dammed and when asked he stated that no brine has been on site for 8 weeks and the cellars are pumped regularly. He then stated that 8 tons of ice melt had been released during the winter on the pad for ice control. I then met with Jeff Wood, Waste Management Coordinator from Hess and Denny Furmanek, Project Specialist from Envirotrac Environmental Services took samples from the ditch, under the containment on the north side and from the cellar of the Athens B 4H-5. I then tested the samples with Quantabs and readings on the ditch were at 1864ppm, containment was 2800 ppm and the cellar was greater than 6428ppm. We discussed their plan of digging and lining a sump and then pumping the water from the sump to frac tanks also discussed was the containment of the cellars and mouse holes. |
