0610638282 2016-05-05 Production Wells No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 500 bbl. Production tanks 5 Producing wells 5 Sand separators

1 Horizontal heater-treater 1 Low pressure separator/ Vapor recovery system 1 Combustor 2 Compressors 2 300 Gallon Fuel Cells 2 NGL Production Vessels 1 NGL Compressor 1 NGL Heater Treaters

1392048159 2014-02-10 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 10-3/4 inch diameter casing string. Nomac Drilling Rig # 137 drilled a 14 inch diameter hole to 2005 ft. and set 1998 ft of 45.5 lb casing. O-Tex Cementing Service c/w1250 sks of class H cement at 15.8 lb 1.21 yield displace with 184 bbls and had 48 bbls of good returns.
1537270846 2018-03-15 Preliminary Restoration No Violations On the day of my inspection I found pad to contain 5 properly labeled producing wellheads. Wells are equipped with christmas tree style wellheads. Wellhead pressures read: surface 0 psi, intermediate 0 psi, production 200 psi, and tubing 200 psi. Production equipment includes 5 gpuss, 5 sand seperators, 1 heater treater, 1vapor recovery tower, 1 combustor, 2 compressors, and 12 400 barrel steel tanks. Tanks inside metal containment. Pad outslopes are vegetated and stable. Site meets requirements of )RC 1509.072A for preliminary restoration.
1939145245 2014-02-05 Drill / Deepen / Reopen No Violations Nomac Drilling Rig # 137 is at 258 ft drilling the Surface section of the well. They should be ready this evening and will call when then start running casing in the well.
2124861011 2014-09-23 Production Wells No Violations On the date of inspection I met with American Energy Utica EHS representative Joe Steele to tour the facility. Four of the five wells on the location have been placed into production as of 8-18-14. There are currently eight 500bbl tanks on the location and one heater separator per well. At the time of my inspection one of the two generators was in use.
