0427458697 2013-12-09 Drill / Deepen / Reopen No Violations I am here today to check on the conductor cement. Nomac 138 is moving in today, they have two trucks stuck on the hill and I received a call about it.
0770319064 2014-01-15 Drill / Deepen / Reopen No Violations I arrived today to check on the drilling status of Nomac Drilling Rig # 137 because I had received a phone call at 5:30 this morning to inform me that they would be ready around noon. When I did not hear from them at 2 pm I wanted to check to make sure that they did not forget to call. I found that they are only at 450 ft and should be ready early this evening.
1366850431 2016-05-05 Production Wells No Violations I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

12 500 bbl. Production tanks 5 Producing wells 5 Sand separators

1 Horizontal heater-treater 1 Low pressure separator/ Vapor recovery system 1 Combustor 2 Compressors 2 300 Gallon Fuel Cells 2 NGL Production Vessels 1 NGL Compressor 1 NGL Heater Treaters

1389835155 2014-01-15 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 16 casing string. Nomac Drilling Rig # 137 drilled a 22 inch diameter hole to 643 ft and set 631 ft of casing. O-Tex Cementing Service c/w 1020 sks of class H cement at 15.6 lbs and a yield of 1.2 - Had problems getting casing in the hole and the job ran over.
1391702375 2014-02-03 Drill / Deepen / Reopen No Violations Nomac Drilling Rig # 137 is finished with the well. They will be moving to the # 1-H well next.
1494372433 2018-03-15 Preliminary Restoration No Violations On the day of my inspection I found pad to contain 5 properly labeled producing wellheads. Wells are equipped with christmas tree style wellheads. Wellhead pressures read: surface 0 psi, intermediate 0 psi, production 185 psi, and tubing 200 psi. Production equipment includes 5 gpuss, 5 sand seperators, 1 heater treater, 1vapor recovery tower, 1 combustor, 2 compressors, and 12 400 barrel steel tanks. Tanks inside metal containment. Pad outslopes are vegetated and stable. Site meets requirements of )RC 1509.072A for preliminary restoration.
1649217841 2013-12-11 Drill / Deepen / Reopen No Violations I am on site today per request from American Energy Partners request to meet with me to go over the permit requirements and drilling questions they had for the Jones wells.
1755861209 2013-12-05 Drill / Deepen / Reopen No Violations Express Drilling Rig is on site today to drill the 30 inch diameter conductor hole. They set 100 ft and O-Tex Cementing c/w 250 sks.
1895988969 2014-01-29 Drill / Deepen / Reopen No Violations Nomac Drilling Rig # 137 is at 9148 ft drilling a 8.75 inch diameter hole. They will move to the # 1-H after this well is complete.
1926895854 2013-01-22 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of Nomac Drilling Rig # 137 and found them drilling at 2484 ft dusting while drilling a 9-7/8 inch diameter hole.
