1421254244 | 2015-01-14 | Production Wells | No Violations | I received a call from Chief Simmers at 9:00 PM to inform me he had been contacted by the Columbus Call Center that an accident had been reported to the Harrison Co Sheriffs office. The accident occurred on Addy Rd and he wanted me to find more information about the incident. I contacted Harrison Co Sheriffs office and was given the name of the person who was hurt [ Dustin Bolen} and a contact person and phone number. I found out that Hydro Edge Solutions was pumping water when a line broke. It had pressure on it and burst. The broken line impacted the leg of Dustin Bolen and he was taken to the Hospital at 9:00 PM with injuries. I went to the site the next morning and found that he was treated and released with several stitches and did not have a broken leg as first reported.
