0132611599 2018-11-07 Production Wells No Violations On the day of my inspection I found this pad to contain five properly labeled wellheads. Wells are equipped with chritmas tree wellheads. Gauges show no surface or intermediate casing pressure. Production equipment on the pad includes five gas production units, one horizontal seperator, one compressor, four 400 barrel steel condensate tanks, and a flare. Tanks are in their own steel containment. Production records current in RBDMS. Pad is now owned by Ascent resources and named has changed to Ace.

The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following:

4 400 bbl. Production tanks 4 Producing wells 4 Sand separators

1407849722 2014-08-12 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 24 inch diameter casing string. HAD Drilling drilled a 28-3/4 inch diameter hole to 340 ft. and set 312 ft. of casing. Halliburton Energy Service cemented with 540 sacks of class A cement at 15.6 lb and a yield of 1.20 and had good cement returns
1415672140 2014-11-10 Casing String Witnessed No Violations I am on site tonight to witness the running and the cementing of the 13-3/8 inch diameter casing string. HAD Drilling Rig # 249 drilled a 17.5 inch hole to 1915 ft. and set 1915 ft. of 54.5 lb. casing. Halliburton Energy cemented with 495 bbl. Of class A at 15.6 lb. and a yield of 1.21 Because of the size of the job and all of the trucks and small location Halliburton took several hours to rig up. Once the job got underway Halliburton Broke down and I did not witness the remainder of the job.

Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
1756708311 2014-08-19 Casing String Witnessed No Violations On the date of inspection I was on site to witness an intermediate casing string. Andy Willson and Pat Parsons were the Hess company men on site. The HP 284 rig had reached a total depth of 7896 feet and were preapring to cement the 9.625 inch casing. While I was on site the cement head was not able to hold pressure and a new one would have to be delievered. The job would be delayed several hours. I left the site due to the equipment malfunction.
