0119820909 | 2014-07-10 | Casing String Witnessed | No Violations | I am on site today to witness the running and the cementing of the 24 inch diameter casing string. HAD Drilling Rig # 690 drilled a 28.5 inch diameter hole to 340 ft and set 312 ft of 186.44 lb casing. Halliburton Energy Service is on site to cement the casing with 515 sacks of class A cement at 15.6 lb and a yield of 1.20 circulated cement to surface. |
0156723233 | 2016-05-06 | Production Wells | No Violations | I arrived on location, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well is producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order.
The production equipment on the pad consists of the following: 4 400 bbl. Production tanks 4 Producing wells 4 Sand separators 1 Combustor 1 Compressor |
0240730871 | 2017-09-22 | Preliminary Restoration | No Violations | On the day of my inspection I found the Cadiz C pad to contain five properly labeled wellheads. Wells are equipped with chritmas tree wellheads. Gauges show no surface casing pressure and 600 psi on intermediate casing. Production equipment on the pad includes five gas production units, one horizontal seperator, one compressor, four 400 barrel steel condensate tanks, and a flare. Tanks are in their own steel containment. Production records current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A. |
0422470025 | 2014-07-29 | Drill / Deepen / Reopen | No Violations | I am on site to witness the bond log on the bottom section of the Mine String which has just been installed. The log truck is late and could arrive any time over the next few hours or days. I advised that I needed a copy of the cement bond log if I did not witness it. |
0526049221 | 2014-07-25 | Drill / Deepen / Reopen | No Violations | I am on site today to meet with Kevin Toothman Company Man for Hess Ohio and Mike Roberts Drilling Superintendent. We are meeting today to talk about the best way to make sure that the bottom 50 ft of the Mine String be re- cemented. After conducting a pressure test after cementing the casing on 07/17/2014 it was discovered that the bottom 50 ft of cement was missing. It occurred after the casing could not be placed on bottom to shut of the u-tube effect when the valve was opened after cementing. The cement was moved up with the hydraulic pressure into the open mine void. The meeting today is to talk about using tubing to spot the bottom plug and increase the odds of it staying in place. The problem is not if the plug would be placed correctly but if the rig can run the tubing. It is not set up to run something that small. |
0676158812 | 2018-11-07 | Production Wells | No Violations | On the day of my inspection I found this pad to contain five properly labeled wellheads. Wells are equipped with chritmas tree wellheads. Gauges show no surface casing pressure and 300 psi on intermediate casing. Production equipment on the pad includes five gas production units, one horizontal seperator, one compressor, four 400 barrel steel condensate tanks, and a flare. Tanks are in their own steel containment. Production records current in RBDMS. Pad is now owned by Ascent resources and named has changed to Ace. |
0848987429 | 2014-07-28 | Drill / Deepen / Reopen | No Violations | HAD Drilling Rig # 618 drilled out the cement from 420 ft. to 460 ft. and found it open to 645 ft. Halliburton Well Service is on site today to conduct a pressure test of the Mine String Casing cement from 0 to 660 ft. The well has been pumped up to 300 PSI and it has held. I am not sure that the cement which has been drilled out has not blocked off the bottom of this shoe. Because I am not sure that there is cement on both the top annulus and the bottom annulus space between the casing a cement bond log will need to be completed to show cement has covered this section and that the mine water has been contained. |
1186539838 | 2014-04-03 | Drill / Deepen / Reopen | No Violations | HAD Drilling Rig # 680 is drilling at 80 feet and are down for repairs. They plan to drill and set casing at 100 ft.. |

1206446196 | 2014-07-17 | Casing String Witnessed | No Violations | Onsite this date with Inspectors Mark Moreland, Randy Akins and Brandon Trigg to witness a mine string. HAD drilling has drilled a 22 open hole to a depth of 660 and set 660 of 18 5/8 casing on bottom. Mine void is from 600-610. Halliburton is onsite to cement the mine string. 150 sacks of class A (15.6 ppg; 1.21 yield) was used to cement below the mine leaving 40-50 of cement in the casing. Operator let the cement set up for 1 hour and began grouting the above the mine void using 420 sacks of cement. Casing was centralized and two cement baskets were on the casing at 590 and 570. Cement was viewed on the surface. |

1405604102 | 2014-07-17 | Casing String Witnessed | No Violations | I am on site today to witness the running and cementing of the 18-5/8 inch diameter casing string. HAD Drilling Rig # 618 drilled a 22 inch diameter hole to 660 ft after hitting a mine shaft at 600 to 610 ft. Set casing with float shoe 50 ft below the bottom of the mine. Halliburton Energy will cement the bottom 50 ft with 150 sacks of class A cement at 15.6 lb and a yield of 1.21. There are 15 centralizers on the casing string and two cement baskets. The cement baskets are at 570 ft and at 590 ft. Used pea gravel to open cement baskets then Halliburton cemented the annular space from 590 ft to surface and cemented with 420 sacks of class A which had good returns to surface |
1405963695 | 2014-07-17 | Casing String Witnessed | No Violations | On the date of inspection I accompanied ODNR Inspector Mark Moreland to witness the cementing of a mine string. Refer to Mark Morelands report for further details. |
1406550273 | 2014-07-28 | Casing String Witnessed | No Violations | I am on site today to witness a pressure test on the 18-5/8 inch diameter casing string which was installed as a Mine String after hitting a shaft at 600 ft and the bottom of 610 ft. The casing has been cemented 07-17-2014 and after drilling out the cement the pressure test failed 07-18-2014. It was then cemented again 07-18-2014 and the cement was drilled out and the pressure test failed. Then it was cemented again 07-19-2014 and the cement drilled out and tested again today 07-28-2014 and the test did pass. I advised that a cement bond log be completed and verify that there is cement across this 50 ft section below the mine. After the cement bond log has been completed and the cement verified then drilling can continue. If cement is not in the 50 ft section below the mine then remedial cementing will be conducted until that section is cemented. |
1438914176 | 2015-08-06 | Casing String Witnessed | No Violations | I am on site tonight to witness the running and the cementing of the 9-5/8inch diameter casing string. Nabors Drilling Rig # 36 drilled a 12-1/4 inch diameter hole to 4799 ft. and set 4784 ft. of 40 lb. casing. Schlumberger Cementing Service cemented with 740 sacks of class H lead cement at 12 lb. and a yield of 1.91 and 340 sacks of 15.6 lb. class H cement with a 1.22 yield of tail cement and had 30 bbl. Of good cement returns to surface. |
156723233 | 2016-5-6 0:00:0 | PW | No Violations | 0.50 |
1912063210 | 2014-07-30 | Drill / Deepen / Reopen | No Violations | HAD Drilling Rig # 618 has drilled out the cement from the 3 rd. cement job done on the bottom section of the Mine String 07/19/2014. They drilled out 10 ft. below the bottom of the casing which was set at 660 ft. to 610 ft. A cement bond log will need to be completed on that section before drilling can continue. A log truck is scheduled for today. |
