0174681944 2014-05-29 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8inch diameter casing string .H & P Drilling Rig # 287 Drilled a 17.5 inch diameter hole to 1935 ft and set 1929 ft of casing. Halliburton Energy Service cemented the casing with 810 sacks of class H cement at 15.8 lbs and a yield of 2.31. Had trouble with the installation of the shoe track and ran over 2 hours. Had trouble with the casing and ran over and then had to circulate bottoms up. Did get cement returns to surface.
1545233148 2018-12-18 Production Wells No Violations On the day of my inspection I found this pad is now owned by Ascent Resources. The pad is now called the Domino pad. Pad is seperated between a production equipment pad and a wellhead pad. Five properly labeled chritsmas tree style welheads are on location. Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, five GPUs, one seperator, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Surface annular pressure was 300 psi and intermediate annular pressure was 100 psi.
1848673893 2017-08-31 Preliminary Restoration No Violations On the day of my inspection I found the Cadiz D pad seperated between a production equipment pad and a wellhead pad. Five properly labeled chritsmas tree style welheads are on location. Production equipment includes three 400 barrel steel condensate tanks, one 400 barrel brine tank, five line heaters, one seperator, flare, and vapor recovery system. Production equipment is fenced and locked and access the wellheads is also restricted by fence. Production records are current in RBDMS. Site passes preliminary restoration according to ORC 1509.072A.
