I arrived on site and found Tom Bauer of AEU on site. I found that there was oil base mud on the road and advised that the Twp. And County Highway Dept. be contacted. Met on site with Jim Albright from the Co Highway and he advised that it was a Twp. Dept. and to contact them. The spill is 200 ft. long 100 wide. It is from the middle of the drilling pad and down over the hill toward Kennedy Ridge Rd. The road is blocked off and equipment is on the way to contain and clean up the contaminated soil and road surface. |

O-Tex Pumping LLC established circulation with 17bbl of fresh water then pumped 190sks (40.6bbl) of premium Class H cement at 15.6ppg with a yield of 1.2 including 2% CaCl, 0.25lbs/sk of flake and 0.4% C41P (Defoamer). O-Tex then displaced cement with 55.7bbl of fresh water. 2bbl of cement returned to surface and stayed at surface while I was on site. |

1914969812 | 2015-10-23 | Completion Testing | No Violations | Inspector Moreland and I came to the site to conduct a follow up inspection after an accident that occurred the previous day on 10/22/2015, when a semi had punctured the passenger side fuel cell. The semi fractured the fuel cell when leaving the Hamilton. After leaving the Hamilton well site the semi was in route to the Pettay site. The semi was then inspected and stopped at the Pettay well site, were they contained the remaining fuel from the semi. During the duration from the Hamilton site to the Pettay site, it was calculated that there was 25 gallons of fuel lost from the semi. Clean up crews were then called and ordered to take care of the clean up. Upon arrival on 10/23/2015 Inspector Mooreland and I witnessed that the lease road entrance and access points were cleaned up. Also the county road was properly cleaned up, the peatsorb was broomed and all contaminates were stored into a roll off bin to be shipped away. |
