1234442787 2018-01-10 Production Wells No Violations On the day of my inspection I found this pad to contain six christmas tree wellheads. Production equipment includes three line heaters, six gas production units, six vertical sand seperators, twelve 400 barrel steel tanks, two compressors, 2 vapor recovery towers, a combustor, and pipeline equipment. This well had zero pressure on the surface gauge and 500 psi on the intermediate gauge. Production records are current through 2017. Sign needs changed to 34-067-2-1375-00-00.
1417528699 2014-12-02 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 30 inch diameter casing string. Top Hole Drilling Rig # 1 drilled a 36 inch diameter hole to 100 ft. and set 100 ft. of 118 lb. casing. Universal Well Service cemented with 275 sacks of class A cement at 15.6 lb. and a yield of 1.20 Circulated 5 bbl. Of good returns to surface.
1421521536 2015-01-17 Casing String Witnessed No Violations Upon arrival on location I found that the crew had installed 403 feet of 13 3/8 inch casing. O-Tex will do the cementing. Established circulation and circulated 30 bbls of good cement returns to surface. Floats held.
1428404361 2015-04-07 Casing String Witnessed No Violations Upon arrival on location I found the crew had drilled to a depth of 1960 feet of 12 1/4 hole and install 9 5/8 inch casing and will cement back to surface. O-Tex will do the cementing. Established circulation and circulated 30 bbls of good cement returns. Released landing pressure and had 3/4 of a bbl back.
1655909182 2016-07-26 Preliminary Restoration No Violations I arrived on location, where I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells have not been fracked, however the production equipment has been place on location. RBDMS indicated that the well is not in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 2 NGL Tanks 6 Non-fracked wells with night caps

1 Vapor recovery system 1 combustor 1 Low pressure separator

2 heater treaters

1 Line Heater The site passed preliminary restoration in accordance with ORC 1509.072 A.

2126340093 2015-01-30 Drill / Deepen / Reopen No Violations Upon arrival on location I found the rig crew drilling ahead at a depth of 14960 feet. The projected TD is 15700 feet. They have not encountered the loss of drilling mud that they did on the last wellbore.
