0164715868 2018-12-06 Drill / Deepen / Reopen Violations Noted On the day of my inspection I was accompanied by Inspector Jeff Adams. Emergency response coordinator Jason Blocker was also onsite. Patterson 580 has drilled a 17.5 inch diameter hole to 628 feet KB. Mine void encountered from 618-628. This void contained a large volume of gas. The rig has a leak on the airbowl seal and was unable to control gas flow. Pad has been evacuated with only necessary personnel on location. Gas monitoring is occurring on location and rig is waiting for a company to arrive to perform radius gas monitoring. Gas is uncontrolled and not being flared. Chesapeake is planning on drawing gas from the 4 inch valve on the 18.625 casing to divert it away from rig floor to make repairs to air bowl. A complaince notice is issued along with inspection: 1501:9-8-2 (A)(1)

Failure to Notify within 30 minutes for: a Release of gas of any amount; Resulting from a Blow out or Any gas release that is a threat to public safety. This compliance notice requires immediate remediation.

0186652473 2019-05-23 Completion Testing No Violations On the day of my inspection I was accompanied by Inspector Jeff Adams. We witnessed a Profrac frac crew onsite tied into the 1H, 3H, and 201H wells. They are no longer pumping stage on this well. Twelve 350 barrel steel produced water tanks are on location, within their own containment. Tanks had <50 barrels each remaining in them.

0899272915 2018-12-04 Casing String Witnessed No Violations On the day of my inspection I was accompanied by Inspector Jeff Adams. We witnessed that Patterson 580 had drilled a 24 inch diameter hole to 580 feet using a hammer bit. They were having trouble running 18.625 inch diameter casing when we arrived. We left to do to other inspections and returned later to witness cement job. Hallburton was onsite with 1160 sacks of class H neocem cement. Cement weighed 17 ppg with pressurized mud scale and a 1.071 yield. 50% excess cement ran in open hole. Costys ran 563 feet of casing and set it at 565 feet. Kelly bushing is 25 feet and casing top is 2 feet below kb. Shoe track is 45 feet long. Cement displaced with 159 barrels of fresh water. 36 barrels of cement returned to surface with early cement returns weighing 14 ppg. Floats held with 1.5 barrels back to the truck.
0938978299 2019-02-27 Drill / Deepen / Reopen No Violations Upon my arrival on location, I witnessed Patterson rig 580 crew was nippling up to the 1H well. They just skidded off the 3H. They will test the blow out preventer on the 1H. They will then drill out the shoe track on the 13.375 inch diameter casing and continue drilling a 12.250 inch diameter hole to the depth of 2000 feet. This is were they will set the 9.625 inch diameter casing.
0985746180 2019-05-23 Completion Testing No Violations On the day of my inspection I was accompanied by Inspector Jeff Adams. We witnessed a Producers Service Corp. frac crew onsite tied into the 1H, 3H, and 5H wells. Producers is pumping stage 69/94. Stage 69 plug set at 12401 feet, with bottom perforation at 12387 feet and top perforation at 12270 feet. Producers pumped 3000 gallons of 7.5% HCl, 10112 barrels of clean fluid, no barrels of produced water, 198180 lbs of 100 mesh sand, and 132080 lbs of 40/70 mesh sand. Total sand pumped was 330260 lbs. Maximum treating pressure was 10738 psi and average treating pressure was 10192 psi. Maximum rate was 97 barrels per minute (BPM) and average rate was 93 bpm. Break down pressure was 6539 psi at 10 bpm. Initial shut in pressure (ISIP) was 5437. 5 minute ISIP was 5085. Frac gradient of 1.09 psi/ft. The 13.375x9.625 annular pressure is zero and the 9.625x5.5 annular pressure is 40 psi.

1225597198 2019-05-06 Completion Testing No Violations On the day of my inspection I witnessed Producers Service Corp frac crew onsite. During my inspection they are performing an iron inspection and changing valves on the wells. No pumping is occurring. Frac is ongoing between the 1H, 3H, and 5H wells, when pumping. The 13.375x9.625 annular pressure was 0 psi and the 9.625x5.5 annular pressure was 85 psi. There are ten 350 barrel prodcued water tanks onsite in their own containment. Stage 46/94 is perforated.
1318938228 2019-04-15 Completion Testing No Violations On the day of my inspection I witnessed Producers Service Company on location tied into the 1h, 3h, and 5h wells. Stage 9/94 on this well is next for frac. Plug was set at 21461, with bottom perforations at 21450 and top perforations at 21330. 13.375x9.625 annular pressure is zero psi and the 9.625x5.5 annular pressure is 55 psi.

A Producers Service Corp frac crew is on location tied into the 1H, 3H, and 5H wells. Stage 53 is perforated on this well and is next to frac. Pad is not pumping during this inspection because they are doing maintenance. Plug set at 14817, with bottom perforation at 14806 and top perforation at 14686. Stage desgin calls for 330000 lbs of sand and approximately 10000 barrels of fluid. The 13.375x9.625 annular pressure is 10 psi and the 9.625x5.5 annular pressure is 10 psi.

1542751151 2018-11-20 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augured a 36 inch diameter hole to 116 feet. 120 feet of 30 inch diameter conductor casing was run. Lightning Energy was onsite with 286 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 30% percent excess ran. Cement displaced to 110 feet. Cement was displaced with 91 barrels of fresh water. No cement returned to surface. Topped out with 9-sack grout on 11/21 using approximately 13 yards.

1547755183 2019-01-17 Drill / Deepen / Reopen No Violations On the day of my inspection I am onsite to follow up on complaince notice #164715868 for general safety, inadequate blowout control system, and failure to notify within 30 minutes for: a release of gas of any amount; resulting from a blow out or any gas release that is a threat to public safety. On 12/14/18 rig had cemented the open mine void effectively killing the gas flow. Rig moved off location to drill another pad. Patterson 580 will return in starting tomorrow with adequate surface control equipment for expected gas show to continue drilling on pad.

Wireline is running in this wellbore to set plug and perforate stage 24/94. Company man does not have exact depths until they are set. The 13.375x9.625 annular pressure is 0 psi and the 9.625x5.5 annular pressure is 76 psi. Company men on location are Terry Hickman and Brad Wrenicki.
