0012495213 2013-01-31 Production Wells No Violations At the time of the inspection the well was shut in and has not been put into production. The pressure gauges on the annulus show 600 psi. There were also bubbles of gas coming up through the water in the cellar around the well. The corrective action from the Compliance Notice issued in 2011 has not been completed. The inspection was done at the request of Eric Vendel from The Ohio Division of Oil and Gas.
0158333614 2017-07-26 Production Wells No Violations This inspection is a followup inspection on NOVs issued in 2015 to plug this well. This well appears to have had no production since the previous inspection on 2015. The two gauges reading pressure inside the well read at 600 psi and there was noticeable bubbling in the water in the cellar. No Hydrogen Sulfide readings were observed. Open pit is still present. It appears that there has been no activity at this well since .

Contractor estimated that approximately 700 lbs of explosives would be used. 0929939350 2007-08-06 Production Wells No Violations On site to inspect cementing of 1318 feet of 6 5/8 inch surface casing. There were 150 sacks of cement pumped to achieve circulation to surface. Excess water from mud pit was pumped to drill cuttings pit. Pit liner in cuttings pit needs to be repaired where it slipped into pit.
1186516700 2007-07-31 Production Wells No Violations On site to observe cementing of 8 5/8 inch casing. Drilling being done by R&S Drilling from Indiana. Cement supplied by M&R Redi Mix. Rotary drilling with mud was recirculated thru plastic lined pit. Eighty eight bags of cement pumped into annular space of 200 feet of 8 5/8 inch casing did not circulate. Cement was allowed to set up and then checked for top of cement. Returned to site late in day where top of cement was tagged at 20 feet below ground level. Topped off annular space with 11 bags of cement. Discussed discharging fresh water onto ground during next drilling on air rotary thru limestone. Water will be tested every 100 feet in drilling depth. OK to discharge fresh water is landowner consent obtained.
1190038130 2007-09-12 Production Wells No Violations Checked site for restoration. Much of site has been graded and topsoil replaced. A small pit is still open next to well. Service rig on site. Gauge on well shows approximately 50 psi in well. Noted some gas bubbles in water around wellhead. Spoke to landowner Mr. Eickmeier on site. He said that the well will be shot to stimulate production.
1202570876 2015-02-11 Production Wells Violations Noted At the time of the inspection the well was still not been put into production since the previous inspection on 1-31-2013. The two gauges reading pressure inside the well read at 600 psi and there was noticeable bubbling in the water in the cellar. No Hydrogen Sulfide was detected but this is a known Hydrogen Sulfide area so precaution should be taken during any service or plugging work. There has been no response from OGE Energy to previous Compliance Notice #1313376911 issued on 9-8-2011. This well is hereby ordered to be plugged and the land restored by 9-11-2015.
1313376911 2011-08-03 Production Wells Violations Noted Stone lane back to well. 7 casing in steel cellar filled w/ water. Bubbles in water around casing. Casing head w/ guages on both sides. 600 psi on one guage and 0 psi on other guage. 2 3/8 upset tubing in casing head w/ valve on top and shut in. No production lines or lift equipment on well. Large mound of dirt on site and closed pit has settled. Preliminary Restoration due on 7/27/07. Preliminary restoration not passed. No I.D. posted.

2117826985 2018-01-29 Production Wells No Violations FOLLOWUP ON NOV # 1313376911 DATED 8/03/2011 and NOV # 1202570876 DATED 02/11/2015 Well consist of 7 surface casing, 2 3/8 tubing with valve shut in. Gauges are present at wellhead and gauge on the annulus reads 590 PSI. No storage tanks on site. Cellar that was used for drilling is still present and approximately 3.5 feet deep and there is still an open earthen pit visible on site. Slight odor of gas around well but no readings were obtained with gas detector equipment. Stone driveway is accessible from to well. No identification is posted at well and well appears to be incapable of production at the present time.
