0068644682 2008-09-11 Final Restoration Violations Noted An inspection revealed that the final restoration does not meet requirements. The lease road was observed to contain 2-4 ruts starting from entrance to a turn around located at the top of the hill. 1-3 ruts were observed around the well site. Flowline consisting of plastic and steel were observed in several locations around the well site and tank battery. (2) 100 bbl tanks surrounded by an earthen dike were observed along the hillside of Nickel Plate Road. A bare area was observed directly below this dike to include a roadside ditch. Salt crystals were also observed at this location. Quantab placed in fluid showed 6.8 units. A letter had been sent to Mr. Doug Kitchen serving as a reminder that the final restoration would be due on 9/11/2008.
