0671752748 2011-10-17 Final Restoration No Violations Well was located where new highway is being built. Well is covered by a sandstone blanket as per ODOT specification, then covered with 50 feet of fill. Accompanied by Jeff Amos from ODOT.
0715195599 2010-11-24 Plug / Plug Back No Violations Witnessed plugging and abandonment. The plugging contractor, Berry Well Service, states that 7 does not exist in well bore. Records are not accurate as only 8 casing exists. Discussed scenario with MRI 3 and left 8 casing in the well bore to protect freshwater. The cement stayed at the surface. Advised Mr. Tony Kilbarger to leave well bore open for a couple days to monitor cement top. The Ohio Department of Transportation requests installation of Vent & Vault system, due to well being situated in a Right-of-Way for the construction of Nelsonville Bypass. A follow-up inspection will be performed to check status of cement top and leaks.
1160145402 2010-11-19 Plug / Plug Back No Violations Witnessed bottom plug through tubing. Prior to setting this plug, a Parmaco packer on 2.875 tubing ran to approx 1200 was retrieved from well bore. Records did not show this information was ever placed in well bore.
1685896149 2010-11-22 Plug / Plug Back No Violations Witnessed both Big Lime and Berea plugs. Records at this time indicate that 7 casing was never pre-pulled and 8 also exists in hole (mudded-in). Advised company to set plug approx 50 below alleged 7 and to 100 above Berea to ensure cover, if problem were to occur downhole. Note: 4.5 casing ripped at 2197.
1999992020 2010-11-29 Plug / Plug Back No Violations A follow-up inspection was conducted to verify status of cement top in well bore and leakage. No observed leakage or detection with gas meter. It was noted that the cement has dropped since last Wednesday and no longer observed at surface. Fluid in and around outside of wellbore. Placed a stick approximately 5 long inside wellbore and did not encounter material. Mr. Tony Kilbarger would later state this evening that he measured cement between 19-20. Mr. Kilbarger agreed to place approved fireclay in wellbore. Note: cable tool drilled well. Although the well is not leaking, Mr. Cary Betzing with ODOT, 740-753-0261, had requested the installation of Vent & Vault system for the well prior to plugging and abandonment. Mr. Betzing states that the well will be situated in a Right-of-Way for Nelsonville Bypass and difficult to access, if said well were to ever leak. ODOT design for installation calls for casing cut 5 below grade. Therefore, approximately 15 of clay would be placed in wellbore.
