Mr. Nutter had stated that he never owned this well and that Mr. Rick Mickle was the owner. Currently, the well is under Mr. Nutters bond. Mr. Nutter was instructed to talk with Mr. Mickle about the required change of ownership form. Received a telephone call from Mr. Mickle. He states that this well was producing house gas. |

When weather conditions allow, topsoil and seed will be used to fill in the area of soil removal. |

As the company excavated the soils, a field test was performed to evaluate Cl- concentration. Approximately 4 - 5 of soils was removed from this location and taken to Mr. Doug Kitchens shop for compost pile. The water table at this location was relatively high, which limited the amount of excavated soil. Furthermore, a Public Water Supply Line was routed through this location along with nearby Southeastern Natural Gas Line. Chieftain Energy placed calls to the respective parties for safety purposes.A 20 x 10 x 4-5 was excavated today. The source of the leak was from existing annular disposal line that was buried. Chieftain Energy repaired this section of line prior to received complaint. The soil was stockpiled today and will be removed tomorrow when, transport equipment arrives at the location. |

No leakage at time of inspection. The trees ranged from young to mature growth and approximately forty may have been impacted from the release. The majority of impacted trees are located along creek bank and next to black plastic. Overall distance to creek approx 50. The first location (closest to well) measured 100 6 (length) x 15 -20 (approx width). The second location (closest to tank battery) measured 77 x 11-25 (approx width). High Range Quantabs were placed at this location. The test was stopped after 7.2 units, which equals 2790 ppm (mg/L) Cl-. Note: The US EPA recommended value is 250 ppm (mg/L) Cl-. A voicemail message was left for Mr. Doug Kitchen, on his cell phone, today at 3:10 p.m. of complaint received, inspection results and time frame of two weeks for cleanup. |

Therefore, only 2 - 3 of soil will be removed. The same procedure performed on second location will also be applied to this location. Note: Mr. Doug Kitchen agreed to a non-compliance for non-related wells today. Mr. Kitchen will provide a letter stating requirements and timelines. |

Informed Mr. Kitchen that AD would be disconnected tomorrow and agreed to placement of meter instead of flare due to potential gas sales. Note: snow cover at time of inspection. |
