0741106062 2010-12-30 Administrative Inspection No Violations Mr. Ron McBride called this morning at 9:43 am to state that yesterday, Mr. Doug Kitchen accompanied him to this well location. He confirmed flowline and electric remain on property. Mr. McBride states that Mr. Kitchen feels he turned in a landowner waiver and well plugged approx 20 years ago. He was informed of database records showing issuance of 1999 plugging permit however, no plugging report or waiver were located.

Chieftain Energy will be contacted and informed that plug report is needed. Files will checked for landowner waiver, due to electric pole & line observed along with flowline.

2026859989 2012-09-20 Final Restoration No Violations Crews have been removing flow line, and cleaning up the site to pass final restoration. I received a call from Mr. Bill Deible at 8:17 am. He stated that the neighboring property owners have been accusing he and his helper of stealing the pipe from the site and someone had left a note for them once again accusing them of stealing pipe from the site. While on site, a tan Ford Escape drove by slowly and a woman yelled from the vehicle and said that she was going to call the sheriff. At that point I contacted Detective Ed Downs of the Hocking county Sherriffs Department and notified him of what was going on and he stated he would take care of it.
