0084917134 2009-06-01 Production Wells No Violations FOLLOW UP TO NOV # 1245254154 A complaint was received alleging non-production, pipeline and electric line burial. An inspection revealed that the well contains a pumping unit with a motor and no belts. A separator and compressor next to the well were found not connected. No flowline attached to the pumping tee, but a section of steel line was observed to be routed from this location to tank battery. The valve on well head for 4.5 casing was observed open with attached 1 black plastic with tee. Facing north the line was traced towards the west through an area containing thick brush/vegetation and found connected to a drip. The line from drip is then routed to a residential dwelling. A second 1 line was traced from tee found routed to another drip at tank battery location. Two lines at this location were found disconnected (Permit No. 3076 included). No identification observed at the well (common tank battery) and has faded/not legible at tank. Approximately 300 of gas line needs was found above the ground. Approximately 50 of electric line was also found above ground near the tank battery location. At this time, the electric appears to have been turned off. Additional production equipment include two 100 bbl tanks with separator and knock-out. The separator was found approximately 10 away from tank and has been disconnected. Earthen dike at tank battery location does not contain a front wall. The landowner further states that lease requires burial below plow depth. Note: Paramount Gas has commercial sales line routed along property, but no connections were observed. Tap location near tank battery consisted of 2 black plastic routed from drip not connected and valve closed. No meter observed.
0145284296 2010-11-30 Administrative Inspection No Violations The well owner, Mr. Bruce Kelly, was contacted today by telephone at 5:06 pm regarding non-compliance with Notice of Violations issued on Lucas wells. Mr. Kelly was instructed to provide identification at the common tank battery and at all three (3) Lucas wells; advised that no dike wall exists for the front of tank battery; two out of three wells are idle, and credit for production (house gas) would be granted for well closest to landowner house (P#3245). Mr. Kelly requests additional time for compliance. Asked how much time he would need, Mr. Kelly replied by stating Spring 2011. Mr. Kelly was advised that request would be discussed with Mr. Robert Stonerock, South Region Oil and Gas Supervisor.

0447132650 2017-10-25 Production Wells No Violations This inspection was conducted in regards to a phone call placed to the Incident Notification One-Call System. The caller reported that the company was not taking care of the wells on her property. The well owner Mr. Justin Kelly was at the tank battery when I arrived. The well was equipped with a pumping unit and motor. This well produces into a shared tank battery with API#3076 and API#3246. The tank battery consists of (2) 100 barrel tanks, (1) knockout tank, and separator inside an earthen dike. Identification was observed at the well and tank battery. No violations were observed. Time of inspection: 3:05 PM.
0455999630 2010-01-05 Annular Disposal No Violations Received a copy of run ticket from 10/14/2009 for oil sales showing the Lucas #2 & #3 wells. A follow-up inspection revealed that a new 2 black plastic line has been routed from the casing side of this well to the casing side of Lucas #2, Permit No. 3246. Valve on the casing side was observed 1/4 open. This well as previously noted appears to be providing house gas. Production lines have not been connected to the common tank battery or separator. No ID has been placed at the well. ID is now present at the tank battery.

0535555826 2008-03-13 Production Wells Violations Noted An inspection revealed that two of the four 100 bbl tanks (saltwater & oil) are corroded and leaking at the bottom. Oil and brine was observed around the tank battery location. This well does not contain a dike. As a result, contamination and pollution has washed over the hill onto the lease road. An SPCC dike will be required for the tank battery, in order to prevent future occurence. No contamination or pollution was observed off-site. Also noted was that the lease road will need to be graded and identification provided at the well and tank battery (common tank battery). The company was working at the time of the inspection. The owner, Mr. Bruce Kelly, was contacted by telephone on 3/13/2008 and informed of the inspection results and requirements. Mr. Kelly was questioned about production records for this well. He states that this well provides house gas.

He states that they will probably paint the tanks and before installing new identification. Asked if service rig would be needed, he states that they would just have to hook up wells. Currently, all three wells are not connected to tanks. One of which produces house gas. This work will be done within next week along with seed and straw for the dike.
