0115849050 2017-07-06 Production Wells No Violations I was requested to attend a meeting with the landowner, OEPA and DOGRM Emergency response . We walked the creek and checked the pond. OEPA found a sheen of oil in the creek. They suggested to Petroxs representative the sheen be collected using several different methods.
0171869019 2018-06-12 Production Wells No Violations I received a request from ODNR emergency response coordinator Mr. Scott King. Mr. King asked me to inspect this well. The landowner Ms. Karen Ward is concerned there is crude oil remain in the soil that forms the banks of the stream and the soil surrounding the area around the pond. I walked the stream and the banks of the pond with Ms. Ward.

I observed water inside the dike and did not contain any oil on top of the water. I pointed out the straw that needs to be removed to Mr. Walters. 0710345499 2017-06-27 Production Wells No Violations Petrox has hired MH Oil Field Services to clean up the oil spill. They have removed the contaminated soil from the lease road. They used an excavator to allow the water to move in the stream that was contaminated with oil up stream of the pond. Brooms were used to move oil to a central location and collected. I sampled a spot in a hay field that the landowner believed was crude oil. The sample was taken to ODNR lab in Cambridge for analysis.

1498406985 2017-06-25 Production Wells Violations Noted I received a call from a producer of oil and gas in Hocking County. He had heard of an oil spill and informed me of the spill. This happened on 6-24-17. I contacted ODNR emergency response. They responded to the spill on 6-24-17. I arrived on site today and determined the well belonged to Petrox. I met Petroxs pumper John Walker on site. He had a water truck on site by 10:30 AM. The spill started when the tank overflowed. The oil exited the dike and ran down a lease road where it entered into a small water way. This water way feeds a pond. The oil entered into the pond. Absorbent diapers are being used to remove oil from the creek and the pond. The oil in the dike was removed using the vacuum truck. The spill will be monitored until the oil is removed.
1498739822 2017-06-29 Production Wells No Violations I met with Petroxs pumper to discuss how to remove the brown scum remaining on the pond and in the creel. This substance will not adhere to absorbent material designed to remove oil. The contractor cleaning up the spill MH Oil Field Services suggested collecting the material with a boom. Once the material is collected in a boom, it could be removed using a vacuum truck.
1509979377 2017-11-06 Production Wells No Violations I received a email from OEPA, they stated they had received a complaint from the landowner stating they were not happy with the clean up of the oil spill. I contacted OEPA representative Marco Deshaies on 11.3.17 and asked to meet with a member of his staff. I met with OEPA environmental specialist Kerry Neil on site at 0950 hours. I explained to Kerry what had transpired since the spill was discovered. She inspected the portion that is considered waters of the state. I reinspected the portion that was the dike and lease road. I did not witness any free oil in the dike or the lease road. This satisfies the compliance notice.
1539697481 2018-10-16 Production Wells No Violations During my inspection I observed the well, tank battery, and stream that was impacted from a tank battery spill in 2017. The well was equipped with a pumping unit and motor. It was not pumping during the inspection. The tank battery consists of (1) 100 barrel tank inside an earthen dike. I observed 6 absorbent pads placed just outside the dike of the tank battery but I did not observe any oil on the absorbent pads or in the immediate area of the where the pads where placed. An inspection of the inside of the dike revealed no oil as well. Inspector Kip Thompson issued a compliance notice to Petrox on 6/25/2017 as they were in violation of ORC 1509.22(A). Inspector Thompson stated that the company was in compliance with ORC 1509.22(A) and resolved the compliance notice on 11/6/2017. As previously mentioned the tank battery spill in 2017 did impact waters of the state. The Ohio EPA oversaw the clean up of oil from this stream. During my inspection of the stream I observed approximately 25 absorbent pads in the stream. I did not observe crude oil on the absorbent pads. I took several photos of the stream during my inspection. Time of inspection: 8:10 AM.
1540569928 2018-10-26 Production Wells No Violations An inspection of the tank battery and dike area revealed no saturated oil. As mentioned in my inspection report dated 10/16/2018 I observed 6 absorbent pads placed just outside the dike of the tank battery but I did not observe any oil on the absorbent pads or in the immediate area of the where the pads where placed. I did observe an area approximately 10 feet by 8 feet in size that was covered with straw just outside the dike. Some of the straw was dark in color and had a slight petroleum odor. I contacted the well tender John Walters at 2:17 PM and asked about the absorbent pads. Mr. Walters said he did not put them there nor did he put the pads down that I observed in the stream. I advised Mr. Walters that the straw should be removed from the area outside the dike. Time of inspection: 10:45 AM.
1553172614 2019-03-21 Production Wells No Violations I met with the pumper John Walter and Terry Irvine who is an insurance adjuster with Irvine & Associates at the tank for this well. John requested for me to inspect the dike area for oil. John said he has observed new absorbent pads placed in the stream below the tank that he did not place there. John believes that the landowner put them there as they might be assuming that oil is leaking from the tank. I did not observe any oil within the dike or outside the dike for the tank. John keeps a written record of the fluid level in the tank and said that he has not noticed any evidence of a leak. I observed what appeared to be the same absorbent pads as I mentioned during my inspection on 10/26/2018. The absorbent pads did not show any evidence of oil. They did appear to be absorbing iron that is leaching from the ground. An inspection of the stream from a distance of 30 feet appeared to have two fresh absorbent pads placed in it. 1602280943 2019-05-21 Production Wells No Violations I responded to a flow line leak at this well location today after being contacted by the pumper John Walters at 7:00 AM. My inspection revealed that the flow line allowed oil to leak and travel approximately 35 feet away. The oil did not enter a body of water. The amount of oil leaked be less than 5 gallons. John Walters met me at the well location at approximately 8:00 AM. John advised me that Ohio Pump in Logan will bring a mini-hoe in to excavate contaminated soil on 5/22/2019. I took several photos during the inspection. The well owner shall clean up oil within 5 days. Time of inspection: 7:45 AM.

1913537268 2017-06-26 Production Wells No Violations I met with the landowners concerning the oil release. They are concerned about the amount of oil that has soaked into the ground. They are also concerned about the oil that entered into their pond. I was shown a area in a hay field that the landowner claimed was crude oil. Upon touching the film, it dispersed without a sheen. I contacted district supervisor Eric Conrad to obtain permission to sample the material and have the ODNR lab analyze the materiel. This will be done on 6-27-17.

This area is before the pond. I checked the pond and did not see any oil in the pond. The process of collecting the remaining oil will take time. Petroxs pumper is checking the diapers daily and removing and replacing as needed.
