0142326383 2018-09-30 Production Wells No Violations I arrived at the tank battery at approximately 4:00 PM after being notified of an oil spill that impacted an intermittent stream which is a tributary of Scotts Creek. I met OEPA On Scene Coordinator Trevor Irwin at 14233 Nickel Plate Rd. prior to arriving to the tank battery. Mr. Irwin had placed containment booms in the stream at these coordinates N39.52015 W--82.40459. This was WP 818. A closer inspection of the stream revealed the source of oil was leaching from the bank of the stream and entering the water. The oil was leaching from the bank of the west side of the stream which is the same side that the tank battery for the Weis #1 & #4 wells is located. I discussed the possibility with Mr. Perko that the oil may be entering the ground via a hole in one of the bottom of the tanks. We both agreed this was the most likely source of the oil. The distance between the tank battery and the stream is approximately 90 feet. I contacted Ms. Marti Weis via phone and advised her that it appears the oil is coming from her tank battery. 0458806762 2018-10-18 Production Wells No Violations During the inspection Clint Poling arrived with his vacuum truck to pump fluids from the trenches. Scott Corder attempted to remove the 100 barrel brine tank with his dozer but it was revealed there was still too much fluid in the bottom of the tank. The tank started to leak contaminated material when it was moved by Scott. Scott scooped up the soil that came in contact with the contaminated material with his mini hoe and placed it onto a plastic lined pit at the tank battery. Clint Poling pumped the remainder of the material in the 100 barrel tanks off into his vacuum truck. Marti Weis is going to have the tanks steamed so that they can be hauled to a scrap yard. Jake Glascock was present during the inspection. Time of inspection: 9:30 AM.
0476157064 2018-11-30 Production Wells No Violations I met with Jake Glascock and Trevor Irwin at the tank battery today. No changes have been made since my previous inspection. The trenches were still open. The plastic 2100 gallon tank that was brought in to recover fluid from the trenches is still on location. The tank is completely full of fluid. 0922957145 2013-01-11 Production Wells No Violations Stuffing box is leaking and has caused an area 2 feet wide by 8 feet long, emanating from the well head, to be contaminated. There is no identification at the well. I spoke with Ms. Marti Weis in person about it and she informed me that she has new stuffing box rubbers to fix the leak and she plans on making a sign for the identification at the well. She also stated that she would remediate the contaminated soil in place using lime, fertilizer and a tiller. The tank battery is shared with the other well on the lease and is in compliance.

1079806425 2018-10-26 Production Wells No Violations My inspection revealed that the 100 barrel tanks have been removed from the tank battery. There was no personnel at the location during my inspection. The soil at the tank battery will need to be removed. I observed the fluid level in the trenches. The fluid level was low but I did observe oil in the longer trench. Absorbent pads were laying on top of the water in the trench and need to be pulled out and replaced. 1083317963 2018-10-15 Production Wells No Violations During my inspection I observed the trenches that were dug to catch the oil before it enters the stream. I observed oil and water in the bottom of both trenches. The fluid level was not close to overflowing either trench. During a phone conversation with Ms. Marti Weis on 10/11/2018 Ms. Weis informed me that she was pumping the water out of the trenches onto the ground. Ms. Weis was previously told to pump the water back into a tank by Jake Glascock. I told Ms. Weis that she needs to be pumping the water and oil into a tank. 1362701876 2018-10-04 Production Wells No Violations I conducted a follow-up inspection today to check on the clean up progress. OEPA OSC Trevor Irwin arrived during my inspection. Clint Poling also arrived during my inspection. A plastic 2100 gallon tank has been brought in to pump oil into from the trench if needed. The trench that was dug parallel with the stream has been extended to a total length of 115 feet on 10/2/2018. A second trench was also dug on 10/2/2018 in between the first trench and the stream. The second trench was approximately 35 feet in length and also contained oil. Oil will be pumped out of the trenches as needed. A second siphon dam was installed yesterday at the culvert under Weis Rd. Clint Poling will be withdrawing oil with his vacuum trucks from both siphon dams as needed. 1542206286 2018-11-14 Production Wells No Violations I conducted a follow-up inspection on the clean up progress today. I met with Larry Mason who was contracted to removed contaminated material from the tank battery area. The contaminated material was removed and hauled to a landfill on 11/12/2018. The clean up at the tank battery has been completed. Mr. Mason wanted to meet with me to go over the work he had done so far. I advised him that my recommendation would be to apply clay within the new dike prior to setting the tanks. Mr. Mason said he would discuss that with Marti Weiss. I checked the trenches for oil during the inspection. I did not observe any oil within the trenches but I did observe approximately 2 feet of water that needs pumped out. I spoke with Marti Weiss on the phone after my inspection and discussed what needs to be done to be in compliance. Time of inspection: 8:00 AM. Weather: 26 degrees F, Cloudy

The dike for the tank battery will need to be built up to contain the capacity for two 100 barrel tanks. I did not observe any oil in the trenches but they were approximately half full of water. The water will need pumped out before the trenches can be filled back in. I called Marti Weis and discussed the inspection with her. I advised Marti that the tanks need to be set back in the dike and hooked up to the wells, the dike built up, and water pumped out of the trenches and then backfilled to be in compliance. 1555522289 2019-04-17 Production Wells No Violations My inspection revealed that the trenches that were dug to contain oil have been backfilled. I observed a 100 barrel tank and 2100 gallon plastic tank within an earthen dike. The plastic tank that was used to pump oil into from the trench was still sitting next to where the trench was dug and was full of fluid. I contacted Marti Weis during the inspection and advised her that the tank needs to be emptied. I also advised Ms. Weiss that identification needs posted at the tank battery. She told me she is going to paint the 100 barrel tank and will post identification after the tank is painted. Ms. Weis said she should have the plastic tank emptied and identification posted within 2 weeks. The well and tank battery are now in compliance. Time of inspection: 1:00 PM.
