0001754705 2007-03-27 Plug / Plug Back Violations Noted On 3/27/2007, a site visit revealed no activity at the well. Attempts have been made to contact Paul Grims shop. No calls have been returned. Paul Grims Company appears to have broken an old production line. A significant amount of C&P was observed, in a low lying area, below the well location. It has been noted that oil gradually rises to the surface. This company has been instructed on several occassions to pump contaminated water into the pit and remove oily soil. Also, noted was that the pit liner needs replaced and fluid pumped from the pit. No change was observed with well plugging status. Company is still having problems running tubing back into the well.

Mr. Paul Grim stated that his crew has been working on the well. Mr. Grim was informed that previous inspections have revealed no activity. Mr. Grim was asked for a pipe talley sheet. Initially, the shot/rip depth was 600 feet and would later change to 784 feet per Mr. Paul Thompson on March 8, 2007. Mr. Grim stated that he did not have a talley sheet on the recovered casing, but would provide one if needed. Furthermore, he stated that if I am a man of my word, I will stand by the initial depth of 600 feet. Mr. Grim stated that he would like to move his service rig onto another Bozett well and begin plugging. Mr. Grim was informed that the current well will need to be plugged before the rig is moved. Mr. Grim was again informed that a power swivel should be utilized on this well. Mr. Grim stated that he spoke with Mr. Kenny Wright about borrowing this tool, but didnt want to damage someone elses equipment. The contamination and pollution below the well site was again brought to Mr. Grims attention. Mr. Grim was informed that more effort will need to be placed on this well. The landowner, Mr. Jeremy Adams, would like for the wells to be plugged and company removed from the property.

The source of this contaminant appears to the fluid contained inside the well which continuously migrates to this location due to the depth. The impacted area is approximately 20 feet below the well location. No activity observed at the well site. The pit liner has not been replaced and contaminant removed.
